Reporting to the Director of Operations & Implementations, this role is responsible for supporting central operations and leading new market implementations and supporting an array of operations enhancement work for existing markets. ConcertoCare care teams are multi-disciplinary and provide comprehensive, in-home primary care or wrap-around services to homebound patients and seniors in both a mobile and virtual environment.

Under the implementation umbrella, this role will also work cross functionally and externally with ConcertoCare’s clients to own and drive implementations in new markets via contracts with health plans, provider groups, and health systems in a value-based care model. While there is a foundation for new market implementations, there will be opportunities to launch additional enhancements at the implementation go-live.

Under the operations umbrella, this role will work cross functionally to identify, scope, execute, and sustain operational changes which will vary in lift, complexity, and cross functional needs. Furthermore, this role will establish and monitor market level practice operations performance metrics, providing ongoing support to improve operational design and efficiencies based on results and the success of the model of care established.

RESPONSIBILITIES:

Implementations

  • Supports new market implementations with various health partners around the U.S. by working cross functionally with internal and external stakeholders including legal/compliance, clinical, operations, IT, HR/staffing, marketing, finance, etc.
  • Leads internal and external implementation teams through effective project management to ensure all tasks are done on time, barriers are worked through, and positive relationships with the customer are maintained
  • Develops customized content for each new market based on contractual requirements and specific market and customer needs
  • Supports the management and growth of customer relationships in a handoff from the Growth team
  • Transitions implemented new markets to a local dyad (operations leader plus local medical director) after roughly 60 days
  • Identifies and executes improvements for the end-to-end implementation process including handoff points and improves documentation for the next market launch

Operations

  • Assesses care team operations across multiple states and clinical product offerings to design a standard operating model and implement utilizing proven quality improvement methods
  • Drives project and change management support for the defined improvement projects from scoping through execution and ensuring successful sustainment of change
  • Provides infrastructure and education to empower care teams to achieve desired clinical transformation outcomes throughout the patient care journey
  • Works directly with care teams to build relationships, ensure deep understanding of opportunities via quantitative and qualitative means, ensures effective education and reporting in achieving transformation and performance improvement goals
  • Utilizes an array of data to identify operational workflow opportunities to successfully communicate, build, and execute improvement plans at the care team level that align with overall organization key performance indicators

QUALIFICATIONS

  • Minimum Bachelor's Degree
  • 6+ years’ experience working in healthcare related field required. Healthcare consulting, health plan, and/or practice operations experience preferred
  • Thrives in autonomous work environments where managing ambiguity is critical for success
  • Actively looks for ways to solve problems and is energized by new challenges and learning
  • Effective communicator with demonstrated experience partnering with customers
  • Detail-oriented with combined with the ability to see the bigger picture and help others along the way
  • Demonstrated ability to work cross collaboratively with an array of internal and external stakeholders ranging from front-line clinical to operations to C-suite
  • Skillful in project managing to completion and highly proactive, self-starter mentality
  • Willingness to travel as it add values (which will vary by market and customer) up to 30%


COVID-19 Vaccination Policy

Our COVID-19 Vaccination Policy is aligned with the CMS Omnibus COVID-19 Health Care Staff Vaccination Rule that mandates providers who work in Medicare and Medicaid certified facilities to be fully vaccinated to best care for our frail and elderly populations. Based on this CMS requirement, ConcertoCare requires all “frontline workers” to be fully vaccinated.

Base Salary/ Wage Range $130,000 to $150,000 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
