Monday- Friday 40 hour position. Position is in Valatie.

Med Transporter/ DSP Position:

As the Medical Transporter you will be responsible for scheduling the medical/dental appointments for the people we support in a timely fashion and provide transportation to the appropriate health care providers. As well as the responsibility for documentation and excellent communication between the Nursing and Residential departments regarding schedules and appointment information.

As a Direct Support Professional, you will be instrumental in providing daily support for adults with Intellectual Developmental Disabilities. You will be an essential part in assisting in achieving goals, promoting independence, encouraging socialization, and providing opportunities for community integration.

Expectations :

  • Schedules all medical and dental appointments prior to their due date
  • Transports clients to all medical, dental & clinical appointments.
  • Ensures the medical consult accompanies Individuals on all appointments.
  • Ensures new prescriptions are verified with the primary nurse.
  • Works with Nursing department regarding all appointments, schedules, appointment information and medications
  • Fills in as DSP when needed or when there are no appointments scheduled
  • Ensure the health, safety, and well-being of the people within the program.
  • Aide in the Individuals completion of daily and long term goals.
  • Plan activities within the community.
  • Actively engage in daily activities within the residence with the individuals.
  • Ensure that the residence is maintained with a high level of cleanliness and respect for the people that reside there.
  • Assist with personal care and health-related needs.
  • Follow and implement agency practices in accordance with each Individuals Individual Plan of Protective Oversight.
  • Follow all NYS and OPWDD regulations in regard to working with IDD populations.

Experience:

  • Working with IDD/ Mental health population preferred
  • CNA or HHA preferred not required

Requirements:

  • High School Diploma or GED
  • Pass all NYS and OPWDD background clearances
  • Must have clean NYS driver’s license that meets Insurance Agency requirements
  • Attend a week and a half orientation in Red Hook
  • Must be med certified or become certified (to keep position)
