GENERAL RESPONSIBILITIES: The CTH Coordinator is expected to coordinate all residential facility and program related efforts with the immediate objective of utilizing and further developing maximum individual potential towards meeting the needs of all individuals served by the home.

PERFORMANCE RESPONSIBILITIES:

1. To recruit and hire prospective caregivers.

2. To obtain and maintain current all documentation required by DDSN.

3. To monitor all individuals and their home environment.

4. To make on-site visits to ensure compliance with all state regulations at least weekly.

5. To complete a PCP Plan within 30 days of initial placement and annually thereafter along with the Service Coordinator.

6. To report unusual occurrences/critical incidents and alleged abuse incidents immediately.

7. To coordinate all community support services when needed.

8. To conduct a monthly review of clients finances.

9. To assist in coordinating visits with natural family.

10. To complete monthly progress notes assessing individual’s skills and/or needs as well as progress on objectives and overall status.

11. To coordinate individuals PCP Service Plan and objectives as well as all other supporting documents (i.e. Assessments, Financial Plan, etc).

12. To oversee reconciliation of weekly work orders with the Maintenance Technician.

13. To supervise vehicle upkeep and report problems to the Maintenance Technician.

14. Assist in the supervision of DCS responsibilities and job duties in CTH II homes. Directly supervise the Facility Coordinators and the responsibilities of the Coordinator.

15. Review DCS Performance evaluations that should be completed by the Facility Coordinator.

16. Monitor each shift monthly as deemed necessary or requested by the VP of Operations or the President/CEO.

17. To ensure resident items are accounted for all times. To ensure inventories reflect accountability of all items. All items inventoried at least quarterly.

18. Monitor Behavior Management Programs and document progress weekly.

19. At least monthly inspection of CTH II homes to include health and safety and other related issues.

20. Oversee of Direct Support Staff work schedule for CTH program.

21. Oversee all medical appointments for CTH residents. Arrange for CTH II Facility Coordinator to accompany when needed, if a CTH nurse is not available.

22. Accompany individuals to appointments if staff is not available.

23. Hold monthly staff meetings.

24. Instruct “building for the Future” training or arrange the instructor with a trainer.

25. Arrange/Instruct in-services for CTH staff as needed.

26. Complete paperwork as required by SSA and DSS (Medicaid and/or SNAP).

27. Oversee prepared time reports for payroll department weekly.

28. Facilitate all annual inspections in CTH residences.

29. Oversee the purchasing of household cleaning supplies and paper goods for CTH residences on a monthly basis.

30. Monitor medicine disbursement in CTH residences and day program.

31. Set up new CTH residence (i.e. furnishings) paperwork and manuals.

32. Implement individual training objectives for each CTH resident to help facilitate greater independence.

33. Supervise collecting of data on training objectives and update programs as needed.

34. Complete Attendance Monthly and forward to SCDHHS.

35. Assist CTH staff in planning social activities for residents.

36. Serve as back-up training instructor as needed.

37. Utilize Therap and all its functions in accordance with the agency.

38. Use personal vehicle in the performance of job duties as needed.

39. Oversee the maintenance of monthly personal needs and petty cash funds.

40. Perform other related duties as requested or needed to meet program standards.

HOURS: Forty (40) hours per standard work week.

Job Type: Full-time

Pay: From $37,000.00 per year
