The Counselor is responsible for:
- The coordination of gathering and completing documentation for patients upon completion of their current inpatient treatment with Spectrum Health Systems.
- Works closely with the case management staff to ensure the necessary documentation smoothly transitions from the current inpatient treatment to the continuing care program.
- Maintains binders and bulletin boards containing information regarding initial housing, job searches, and establishment of medical services, social services, education, support services, and services from other community providers that are available for patients to review on the unit.
- Assists in facilitation of group counseling services for patients; preparing necessary documentation including but not limited to photocopying handouts, assisting with group notes when needed.
- LADC I or CADAC certification preferred, but not required.
- At least one year experience working in a substance abuse treatment, case management role or closely related field.
- Minimum of a high school diploma or equivalent
