As a CMH Supports Coordinator and Administrative Assistant, you will function as a Supports Coordinator for 20 hours per week and an Administrative Assistant for 20 hours per week. This individual is responsible for delivering community support, psychosocial rehabilitation, and a variety of other services to individuals with mental illness or co-occurring mental illness and substance use disorders. In the Administrative Assistant role, this individual will specialize in supporting the Community Mental Health contract, along with assisting the billing specialist in ensuring all therapists and interns have met the CMH requirements, tracks all CMH cases to ensure there are the proper documents and authorizations, and assists clients over the phone and in person with scheduling and payment.

This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with agency, federal, and state regulatory requirements.

ESSENTIAL JOB RESPONSIBILITIES

  • Serve as the liaison between clients and Ottawa County Community Mental Health;
  • Ensure clients access the internal and external treatment services and community-based social and economic services required for achievement of treatment goals and maximum independent functioning;
  • Provide targeted case management services to Community Mental Health clients through the Bethany Christian Services contract;
  • Meet with clients in the office, community, and client residences;
  • Maintain own caseload under supervision of the Adult Contract CMH Supervisor and promptly address any CMH client in need of assistance;
  • Assist clients in connecting to various community supports and accessing community resources including, but not limited to: housing assistance, financial assistance, transportation, employment, CLS services, AFC homes, and etc.;
  • Assist clients with coordinating benefits such as: Health Care Coverage, Food Assistance, State Emergency Relief, Disability Benefits, Social Security Benefits, and etc.;
  • Provide timely outreach to clients to establish and maintain engagement in treatment;
  • Assist clients in improving daily living skills and in social and self-sufficiency activities.
  • Coordinate care with primary care physicians, including attendance to medical appointments and advocate with medical community for clients, when necessary;
  • Maintain a list of community resources for utilization by clients and staff and update as necessary;
  • Keep staff up to date on resources that may be valuable to both clients and staff members;
  • Attend Community Case Coordinators meeting in Holland and Grand Haven on a monthly basis and report information back to team as necessary;
  • Actively participate in at least one agency committee;
  • Participate in team meetings with Ottawa Community Mental Health and Bethany Christian Services to discuss and review individual consumer programs and progress and identify issues in treatment for specific individuals;
  • Work collaboratively with Therapists, other Specialists, and Clinicians in obtaining biopsychosocial information and developing, reviewing, and modifying individual consumer treatment plans to improve individual behaviors and life skills;
  • Identify and establish working relationships with community resources for clients, while establishing referral protocols;
  • Ensure that service delivery to clients complies with CMH standard operating policies and procedures, Title X, Medicaid Manual for Providers, Michigan Mental Health Code (PA 258 of 1974), Michigan Department of Community Health rules and regulations and Self determination Initiative, and CARF accreditation rules, regulations and requirements;
  • Maintain case documentation and case notes (both for Bethany Christian Services and Ottawa County Community Mental Health) and ensure the security and integrity of private health information in compliance with the provisions of HIPAA, Ottawa County, and CMH policies and procedures;
  • Prepare all documentation and reports required to maintain compliance with Recipient Rights rules and regulations;
  • Perform administrative task and track all CMH cases to ensure proper documentation and authorization.
  • Greet and welcome visitors upon arrival with a positive attitude, while also checking them in and collecting payment, as needed;
  • Promptly address incoming calls and route them to the appropriate person;
  • Receive and route incoming postal and inter-departmental mail;
  • Complete other duties as assigned.

QUALIFICATIONS:

  • Bachelor’s degree in Human Services or related field of study from an accredited college;
  • Must have a broad knowledge of basic recovery, and treatment principles, concepts, and methodology of social work for individuals with co-occurring mental illness and substance use disorders as acquired through a Bachelor’s level degree;
  • Bilingual in English/Spanish;
  • Possess a working knowledge of various Evidence-Based Practices, behavioral modification protocols, practices, and techniques related to target population;
  • Demonstrated knowledge of professional standards of practice and ethics;
  • Demonstrated knowledge of the principles and practices of medical record documentation and medical records management;
  • Demonstrated knowledge of public and private services and community resources available to the mentally ill and developmentally disabled populations and to individuals with cooccurring mental illness and substance use disorders;
  • Must be able to complete all trainings as required by Bethany Christian Services and Ottawa County Community Mental Health;
  • Excellent verbal and written communication skills, as well as interpersonal skills;
  • Must have an ability to provide excellent and compassionate service;
  • Ability to work independently and exercise a high level of confidentiality;
  • Must be reliable with time sensitive deadlines and tasks;
  • Work well under pressure and adaptable to change;
  • Working knowledge of word-processing, spreadsheet, database, and other necessary software programs.;
  • Computer and typing skills sufficient to perform essential job functions;
  • Active motor vehicle license and maintain a reliable vehicle with adequate insurance coverage;
  • Pass a criminal history screen, including state and local child protection agency registries;
