St. Christopher's has more than 220 pediatric experts on staff, combining top-notch pediatric care with a wide array of pediatric specialties including Cardiology, Ear, Nose and Throat, Gastroenterology, Oncology, and Orthopedics. It houses the only Verified Pediatric Burn Center between New York City and Baltimore and is one of only three Level I Pediatric Trauma Centers in Pennsylvania. Responsibilities
The Community Health Worker is a member of the patient core care team. CHW's serve as liaisons between health educators, communities, social services and other healthcare professionals. Advocating for the needs of individuals and the community, education on the availability of healthcare services and conducting outreach and linking families to resources.
CHW's help patients with complex health conditions navigate healthcare systems.Community Health Worker will primarily be working out in the community with specific target populations (Street Medicine patients). CHW will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.
Qualifications
High School Diploma or GED
Certification as a Community Health Worker preferred
