Community Habilitation services at YAI want to support a person in becoming the best they can be and live the life they desire for themselves.

In YAI Community Habilitation we recognize that each person is unique. Services build on the individual’s strengths and help them achieve the goals they set for themselves. Flexibility and choice are inherent features of the program which make it attractive as an alternative to traditional programming for people receiving the service. Services can positively impact a person’s quality of life by enabling access and opportunity to learn new skills and participate in activities that have been inaccessible because supports were not available. Services through Community Habilitation grow and change as the person grows and changes.

A CHS works one-to-one with a person on a scheduled basis to achieve goals that are related to the person’s desired outcome. Sessions can take place in the person’s home or in the community depending on the service, supports and activities chosen. Services provided through Community Habilitation include:

  • Health skills training
  • Socialization skills training
  • Communication skills training
  • Functional skills training that support a person integrating into the community (i.e. money management, using public transportation)
  • Assertiveness/self-advocacy skills training
  • Community integration and resource utilization skills training
  • Exploring hobbies and interests
  • Skills development training to support job readiness.

A CHS’s responsibilities include:

  • Create/Foster a positive learning environment
  • Teach skills identified by the person supported
  • Identify resources and activities to support the person’s interests and preferences
  • Provide structured training sessions
  • Provide supervision and support as related to safety
  • Utilize good judgement and problem solving to respond to situations/challenges
  • Advocate for the best interest of the person
  • Travel to person’s preferred activities and locations
  • Support the person to develop and maintain relationships
  • Participate in trainings and team meetings
  • Collaborate with team members
  • Maintain a consistent schedule
  • Utilize technology to document daily services and monthly progress

Work Schedule

We ask that successful candidates have at least 12-16 hours available to work. Schedules are consistent from week to week. Upon hire a supervisor works with the new Community Habilitation Specialist to create a schedule based on their availability and the requested schedule of people waiting of services. As you prepare for the interview, this is a good time to think about the time that you have available in your schedule should you be offered a position. Use the table below for list the days and times when you are open to working. During the interview you will be asked to share this information. The Supervisor will use this information to develop a work schedule so please only provide days and times that you can commit to.

Pay: $17.99 - $18.00 per hour

  • 10 hour shift
  • 8 hour shift
  • Monday to Friday
  • Weekend availability
