Community Habilitation is a program operated under OPWDD to provide one-to-one training to people with intellectual/developmental disabilities to develop or enhance the skills needed to live more independently in their homes or in the community. Community Habilitation supports the person's Life Plan and contains activities that are individually customized to help the person achieve his/her goals. It is then implemented through home visits and community activities.

The ideal candidate will have enthusiasm for working with people with intellectual and developmental disabilities while having the ability to provide support and training in areas of daily living, behavioral, socialization and communication skills. Goals will be implemented with techniques of Applied Behavior Analysis. The DSP 2 utilizes support skills for people with moderate to complex levels of need, participates in developing treatment plans, and understands specialized clinical knowledge for person specific needs

Job Responsibilities:

  • Provide physical and cognitive adaptive skill support that is aimed to help the person achieve optimal independence both in the home and the community.
  • Spearhead community inclusion activities / travel training planning
  • Responsible for aiding in helping the person develop relationship building skills
  • Provide guidance in helping the individual access their community while serving as a positive role model in all environments.
  • Engage in mutually respectful partnership/relationships with the individuals, family members, co-workers, and others
  • Support the safety of all individuals in everyday situations
  • Teach/assist with feeding, grooming, and care of any pets that may be in a home
  • Assist consumers with all ADL needs including, but not limited to, feeding, transferring, lifting, toileting, dressing (coats), etc., as per consumer’s protocols.
  • Document Community Habilitation activities on a daily and monthly basis.

Education and Experience:

− A High School Diploma; some college is a plus

− Work experience with intellectual/developmental disabilities, serving as a DSP

− Proven excellence at working with people with intellectual/developmental disabilities

− Knowledgeable and comfortable with ADLs

− Completed all mandatory OPWDD and YAI trainings

− Computer knowledge

− Tuberculosis test mandatory for most positions

Additional Knowledge, Skills and Abilities:

− Exceptional interpersonal, problem solving and communication skills

− Willing to take initiative

− Highly flexible personality type

Pay Rate: $16/hr
