Job Summary:
Provides for safe transfer/escort for PACE participants going to the day center, medical appointments, and on activity trips. The CNA - Transport Aide will assist with the needs of participants including assistance to embark and disembark from a van or wheelchair lift, if required. Assist participants from their residences for transport and serve as communication liaison between the driver and the Transportation Manager. The CNA - Transport Aide will demonstrate the skills necessary to assist frail elders with mobility issue in accordance with PACE program standards.

Job Duties/Responsibilities:

  • Assists in planning daily transportation routes with the Transportation Manager and driver. Participates in formulating and updating care plans to include transportation/escort issues.
  • Ensures that emergency medical information is available on each client in case of emergency. The face sheet includes a current medication list, code status information, medical diagnoses and family contact information.
  • Maintains first aid supplies in case of emergency while in transport. Communicates with the Transportation Manager at the PACE center via cell phone for all emergency needs or concerns that may arise during a transport.
  • According to policy, calls participants before pick-up and to notify if delayed or early for pick- up.
  • Provides physical assistance to each client when he/she boards the bus and disembarks from the vehicles.
  • Ensures that all passengers are secured in their seats and wheelchairs are sufficiently and safely secured before transport in coordination with the driver.
  • Assists with the transport of clients including but not limited to Day Centers, Life Enrichment trips, outside medical appointments, etc. as requested by the Transportation Manager.
  • Identifies service delivery problems and uses good judgment in the solution.
  • Informs the Transportation Manager of “unusual occurrences”.
  • Acts as a first responder in case of medical emergency on the van.
  • Maintains accurate record of daily trips, individuals transported and assistance required. Ensures all necessary information is collected to comply with reporting requirements and submits reports to the Transportation Manager.
  • Provides courier services when necessary.
  • Performs CNA tasks in the home when requested by the Day Center Director.
  • Provides support for Day Center staff when not scheduled for transports including but not limited to assistance with activities, provides ADL support, etc.
  • Utilizes supplies and equipment appropriately and is responsible for maintenance of cell phone according to manufactures recommendations.
  • Maintains a professional appearance: scrubs and PACE apparel are required.
  • Maintains a professional attitude when dealing with unexpected emergencies, outside personnel, changes in workload, and other issues with tact and diplomacy.
  • Recognizes own limitations and seek help and guidance from the Transportation Manager as appropriate.
  • Responds in a positive manner to constructive criticism and serves as a role model for students and staff members. Serves as a role model for new trainees.
  • Adheres to the policies and procedures of the PACE Organization and to the PACE Participant Rights Policy at all times.
  • Protects privacy and maintains confidentiality of sensitive participant and agency information.
  • Attends staff and participant care planning meetings as requested, and department and agency in-services as required.
  • May be required to provide in-home care including personal care and housekeeping services.
  • Works rotating weekends and/or holidays as assigned. Performs related duties as assigned.

Education Requirement:
High School Diploma or Equivalent, Required

Physical Requirements:
Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time.
Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds.
Must be able to assist a 200-250 lb participant, using proper body mechanics.
Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.
Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.
Must be able to complete assignments in variable weather conditions and ever-changing and sometimes allergenic environments in participant homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.

Experience:
1 year of CNA experience, Preferred
1 year of experience working with a frail or elderly population, Preferred

Licensure & Certification Requirements:
Rhode Island Certified Nursing Assistant License (CNA)

Position Requirements:
CPR Certification
Driver's License & access to reliable transportation
