Position: Clinical Team Lead

Reports to: Chief Executive Officer/Supervising Physician

Job Summary

The Clinical Team Lead (CTL) assists the CMO with the oversight of the Provider clinical Operations for all three CFC clinic locations. The Clinical Team Lead functions as a member of the multi-disciplinary health care team within the scope of his or her medical practice, and provides direction to clinical staff at the Centers.

Position Requirements

  • EDUCATION: Current TX medical (or NP or PA or CNM) license and DEA; Current CPR certification.
  • EXPERIENCE: Leadership experience in a clinical setting preferred.
  • KNOWLEDGE/SKILLS: Knowledge of clinical protocols and best practices; able to train and direct others in these practices.
  • OTHER: Travel to regionally assigned sites for on a regular basis as described above, and/ or for training and meetings. Must have reliable transportation, TX driver’s license and proof of insurance.

Duties and Responsibilities

  • Provides Clinical Services
  • PATIENT CARE
  • Provide high-quality, comprehensive, evidence-based primary care to assigned patients.
  • Elicit history, perform physical examinations, and order and interpret laboratory analyses and diagnostic images to provide information on patient’s condition.
  • Develop and execute a comprehensive, personalized treatment plan including health counseling, medication, specialty referral and timely follow-up.
  • See an appropriate number of patients and strive for model productivity in the midst of managing a complicated, chronic, multi-problem patient panel. Serves as an advocate for difficult patients and leads by example, managing them with excellence and efficiency.
  • Coordinate and schedule site provider per diems (in coordination with Site Manager and COO) and ensure mechanism for follow up on studies/labs ordered by per diem providers.
  • Help coordinate continued care for patients who request alternative primary care MD (or providers who seek to transfer primary care of one of their patients away from them due to lack of therapeutic relationship). The CTL is not obligated to assume all the difficult patients.
  • Positively impact patient experience by demonstrating values of Transforming Care including, but not limited to, courteous and helpful behavior and a commitment to quality.
  • Share accountability for overall patient health outcomes, working in coordination with Quality Management efforts, Care Teams, and Population Health staff.
  • Operate to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholders.
  • STAFFING & SUPERVISION
  • Act as resource person for medical staff regarding policies, procedures, various funding sources, etc.
  • Serve as liaison between medical providers and staff in assuring quality care and follow through of important tasks in all departments.
  • Regularly check email, mail, voice mail, and other forms of communication.
  • Assess training needs and promotes developmental activities. Monitor provider quality, efficiency, and teamwork and provides feedback, coaching and mentoring as needed.
  • Participate in training of site Registered Nurses (RNs) and Medical Staff, depending on the topic. Work in conjunction with the Nursing staff/Site Managers to ensure RNs/LVNs and MAs are performing at the highest level in their assigned roles.
  • Conduct on-site interviews for provider candidates. Assist with new provider and new Medical Staff on-boarding/ orientation to site.
  • Supervise, coach, and evaluate medical staff and give input on other staff performance against job description criteria and competency assessment and provides guidance and coaching to develop an individual as well as a unit to highest potential.
  • In conjunction with the CMO conducts timely performance appraisals for medical providers, incorporating productivity metrics, UDS measures and feedback from others.
  • In collaboration with Human Resources, provide appropriate performance feedback, counseling and disciplinary action when indicated and in a timely manner. Maintain records of same and forward to Human Resources as necessary.
  • Intervene in challenging provider situations in collaboration with CMO/CEO and HR. In conjunction with Human Resources, recommend or make decision to hire, transfer, suspend, layoff, recall, promote, discharge, assign, reward or discipline. Promote results supporting equal employment opportunity.
  • Keep staff informed of organizational activities and promotes mission and goals.
  • Visit sites or meets with site managers/medical providers monthly; additional visits as needed or requested.
  • SITE AND CROSS-ORGANIZATION COLLABORATION AND SUPPORT
  • Will provide ECW training for onboarding providers as well as trainings for order sets and templates.
  • Participates in development of clinical policies and procedures and updates.
  • In conjunction with the Site Manager and COO, monitor site productivity. Make necessary recommendations or changes to increase patient show rate, improve patient flow and maximize provider productivity, monitors redesign implementation and results. Use various data sources in making decisions.
  • Participation in all provider evaluations.
  • Lead, attend and/or participate regularly in various meetings across the organization (Leadership, QA/QI, staff meetings, monthly provider meetings, etc.) necessary to accomplish company-wide and site goals.
  • Collaborate cross-functionally with behavioral health, dental and other specialties.
  • Assist in population health efforts.
  • Serve on Quality Management Committee and involved in review and handling of quality management issues/occurrence reports.
  • Working in conjunction with the PCMH project coordinator and Site Manager, implements and sustains the PCMH model, Care Transformation efforts, and Value-Based Care initiatives once initiated.
  • Attend training as determined by the CMO/CEO.
  • Establish and maintain regular contact with CMO, Medical Directors, Site Managers and Human Resources regarding Medical Staff issues, clarifying expectations and expected professional conduct.
  • Support and establish frequent communication and cohesive working relationships with the Site Manager, COO, and other departments by building relationships and by planning and participating in activities together. Serve as a positive role model.
  • Keep informed of organizational activities and promote mission and goals.
  • Maintain approachability, amiability, and trust, serving as general resource/go-to person for clinical issues at site. Intentionally builds morale among employees at site, especially medical providers. Looks for opportunities to affirm good work ethic and maintains open communication with others addressing important issues immediately as they come up. Looks for random opportunities to sit down with individual medical staff and encourage them with real feedback re their strengths, how much they are appreciated, and offers constructive criticism for improvement.

  • Knowledge of medical terminology and appropriate abbreviations
  • Ability to document, with clarity, all information relevant to a patient’s needs
  • Ability to effectively present information in small group situations to patients, Center staff, and the public.
