Summary of Position:

The clinical Relationship Coordinator is a key consultant and liaison for our Trust Administrators.

The Coordinator:

· Collaborates with the organizational team to determine how clients' accounts can be used optimally in their best interest.

· Reaches out to new beneficiaries and/or responsible parties to schedule an initial visit to assess needs, opportunities to enhance the quality of life and medical status.

· Communicates with trust management staff through clinical assessments, reports, conversations with the Administrator, participation in weekly Rounds, and other communications as needed.

· Initiates contact with beneficiaries through calls, e-mails, text and letters, and follows up in most cases with a visit with the beneficiary and/or responsible party.

· May work with family members, agents under health care proxies and/or durable powers of attorney, home-health aides, facility-based caregivers and other responsible parties to make recommendations to the Trust Administrator.

  • Advises Trust Administrator about beneficiary's condition, circumstances and needs, and formulates recommendations about how their trust account may be helpful.
  • Participates in formal weekly meetings and informal meetings as needed with Trust Administrator to communicate clinical information and help to formulate distribution plans for beneficiaries.

· This job requires extensive travel to and from Community Trust's office in Andover or home, and/or to the locations of various trust beneficiaries who reside in other communities.

Primary Duties:

  • Matches beneficiaries with available community services and resources, and may schedule or coordinate beneficiaries' access to funded services and resources.
  • Maintains accurate records of visits, assessments, recommendations, contacts and other documentation relevant to providing support to beneficiaries.
  • Maintains client confidence and keeps operations information confidential.

· Case management.

· Assistance with health insurance, social security, housing, food stamps, fuel programs and other applications.

  • Updates job knowledge through continuing education, professional publications, personal networks and participation in professional as well as maintains updated knowledge of pooled trusts and associated policies.

Qualification:

· Social Services degree.

· Extensive knowledge of community resources available to young disabled people in MA.

Schedule:

· This is a 3 days per week position.

Job Type: Part-time

Pay: $30.00 - $36.00 per hour
