Clinical Quality Coordinator Job at Physicians Group Services, PA
We are currently searching for a Quality Coordinator to join our growing team.
-Monitors daily census from hospitals and payer resources to reduce emergency room utilization and hospital readmissions
-Ensure timely appropriate follow up appointments are scheduled, adhered to as well as work with patients to plan their care from setting to setting while promoting timely access to appropriate care
-Acts as advocate, liaison and information resource for the patient, care team, and family
-Assist with identification of “High Risk” patients and facilitate patient access to appropriate Case/Care Management programs provided by Insurance providers
-Ensure correct documentation, coding, and billing is submitted appropriately
-Provide medication education, review and promote medication adherence
Quality component job duties:
-Ensure appropriate forms are in chart prior to appointment time
-Quality forms completion and submission for all insurance plans
-POC for on-site health screening events. Perform outreach/schedule patients for such events
-Serve as resource/liaison for physicians and staff to complete on-going HEDIS projects
-Collaborate with staff regarding work flow process, improving documentation and abstraction of data to close Quality care gaps
-Ongoing chart review for HEDIS/Quality measures for insurance plans
-Patient outreach including direct patient contact and perform activities to comply with HEDIS/Quality screenings (A1c, urinemicroalbumin etc.) as needed.
-MWOV (members without office visits) outreach/scheduling
-Initiate appropriate dismissal paper work for non compliant patients as needed per CMS guidelines

Qualifications:
-A minimum of 2 years experience in clinical setting
LPN
-Autonomous and demonstrates a high level of motivation and flexibility
-Highly organized with ability to keep accurate notes and records
-Proficient in Microsoft Word, Excel, Outlook email and billing software: Athena
-Knowledgeable of ICD-10 and CPT coding
-Excellent verbal/written communication skills
Job Qualifications
- High school diploma or equivalent required, higher education in health care preferred including medical assistant training or LPN
- A minimum of 2 years experience in a clinical setting
- Autonomous and demonstrates a high level of motivation and flexibility
- Highly organized with ability to keep accurate notes and records
- Proficient in Microsoft Word, Excel, Outlook email and billing software: Intergy.
- Knowledgeable of ICD-10 and CPT coding
- Excellent verbal/written communication skills
- Leadership capabilities
Job Type: Full-time
Pay: $19.00 - $22.00 per year
