Position Summary:


As a member of a Integrated Dual-Diagnosis Treatment (IDDT) clinical case management team, the Clinical Case Manager plays a critical role in supporting a client’s progress as they move through the stages of recovery, and assists them to transition from the behavioral health care system. The Clinical Case Manager is responsible for establishing contact with newly referred clients and developing a collaborative relationship that will facilitate their participation in treatment. The Clinical Case Manager is responsible for maintaining consistent contact with clients, deepening the treatment relationship, assisting clients to identify life goals and linking them to community resources that can support their wellness and recovery. The Clinical Case Manager needs to have the skills, knowledge and ability to shape an effective working alliance, foster hope, build motivation, assess pressing needs, identify barriers to treatment, and link clients to resources to meet their basic needs. The Clinical Case Manager must demonstrate the knowledge and clinical expertise to assess and diagnose, teach clients the skills that will support recovery and enable them to successfully pursue their goals, and to provide clients with regular coaching to apply these skills in their daily lives. Successful candidates will be knowledgeable and experienced employing intervention strategies using motivational interviewing, harm reduction and abstinence-based models of treatment.


Qualifications:


Required:

  • Master degree in social work, psychology, or a related field plus license eligible and registered with BBS with 1 year of experience delivering co-occurring substance abuse and mental health treatment.
  • Clinical skills and experience to assess, plan, develop, coordinate and provide treatment, rehabilitation and support services to SIL clients with severe and persistent mental illnesses.
  • Must be able to communicate effectively both orally and in writing.
  • Must be computer competent
  • Must have a vehicle to use for work duties, a valid California Motor Vehicle Operator’s License, and automobile insurance.
  • Must demonstrate an ability to work both independently and collaboratively as a member of a treatment team.
  • Must be able to maintain a flexible work schedule in accordance with client and program needs.
  • Must have strong organizational skills, as well as, the ability to effectively prioritize competing demands and to meet deadlines.
  • Experience with Medi-Cal documentation standards strongly preferred.
  • Must be able to meet Immigration Reform Act of 1986 requirements.

Essential Functions/Skills:

  • As a member of the clinical case management team, coordinates treatment and supportive services for 25-30 adults with co-existing psychiatric and substance use conditions in all stages of treatment and recovery.
  • Completes initial and annual assessments, ANSAs, and chart authorization process.
  • Provide strength-based, trauma-informed, culturally humble, and non-judgmental high quality services to meet clients’ comprehensive needs, including linking clients to housing, employment, medical, dental, psychological, parenting, legal, transportation, and money management resources.
  • Conducts outreach to engage client in services and treatment, and when appropriate, family members, to support client.
  • Liaisons and consults with community-based organizations to ensure coordination of care.
  • Provides psychoeducation to clients and when appropriate, family members, related to health, well-being, and lifestyle, including symptom management, medication, diet, sleep hygiene, trauma-informed care, and relapse prevention; assists clients in creation of personal WRAP plans.
  • In collaboration with client, formulates client-centered, recovery-oriented, integrated treatment plan: assists client in identifying and articulating personal goals; identifies behavioral objectives and mental health symptoms that may be barriers to those goals; determines treatment strategies and interventions to achieve desired goals; establishes measurable outcomes to determine effectiveness of treatment.
  • Provides ongoing review of client’s mental status, response to treatment, and progress in recovery. Initiates modifications to treatment plan to ensure appropriate interventions are provided in response to changes in the client’s mental status, behavior or stage of recovery.
  • Assists clients in developing individualized transition plans to facilitate exit from behavioral health system or transition to lower level of mental health services.
  • Performs risk assessments, develops crisis management plans targeting problematic behaviors, and delivers effective treatment interventions to reduce risk of harm to self and/or others.
  • Complete consistent documentation that is accurate, timely, clinically useful, and meets Medi-Cal, ACBHCS, and Bonita House standards.
  • Shares on-call crisis intervention duties on rotational basis with other team members.
  • Responsible for producing required number of Medi-Cal billable units per month.
  • All other duties as assigned by supervisor.
