Graham Windham seeks dynamic child and adolescent mental health clinicians to join our growing Care Coordination department. The Clinical Care Coordinator will carry major responsibilities for ensuring that mental and behavioral health services for our children are coordinated with service providers and the right service is provided at the right time by creating linkages and referrals. The Clinical Care Coordinator views each case holistically and values the advantage that all disciplines interconnecting make for far greater outcomes. They believe this modality will not only affect the child/adolescent individually but will have ripple effects on the entire family unit and its stakeholders. They will advocate for the provision of therapeutic services to all children in Foster Care, which meet optimal standards for clinical excellence.

Duties and Responsibilities:
  • Coordinates between Graham Windham stakeholders, primarily physical, mental, and behavioral health care providers, and birth and foster parents to develop a holistic comprehensive treatment plan.
  • Creates goals and outcomes utilizing historical and currently available information in collaboration with the multi-disciplinary team.
  • Attain a comprehensive, working understanding of person-centered treatment planning and availability of behavioral health services to ensure all supports and interventions that are offered to children/adolescents in foster care have been accessed internally and via external community providers.
  • Conduct psychosocial assessments, initial mental health and risk assessments, SOGI questionnaires, etc. to determine clinical eligibility and program appropriateness via Graham’s holistic approach.
  • Gathers collateral information for optimal assessment and planning.
  • Provide mental health supports and therapy such as individual counseling and treatment, substance abuse counseling and treatment, trauma-informed counseling and treatment, family and group counseling, crisis intervention, and/or transitional counseling as applicable.
  • Utilize various EHR (Electronic Health Record) systems for scheduling, uploading, and tracking medical and behavioral health information.
  • Maintains patient/employee confidentiality in the management of protected health information.
  • Documents all counseling and psycho-education services, assessments, reassessments, referrals, and follow-ups in the child’s record/chart.
  • Ensures appropriate signatures and dating of forms and entries including those made in the electronic records.
  • Participates in organizational and service planning meetings as appropriate such as High-Risk meetings and ensures that the best provision of services is offered.
  • Participates in the department’s performance improvement activities and supports department outcomes.
  • Acts as a resource to the Care Coordination department and other agency stakeholders in the child/adolescent's life by: suggesting preventative care, health, and wellness initiatives, and community supports as they pertain to mental health and/or behavioral health concerns.
  • Promote evidence-based wellness, prevention, and treatment practices for a variety of health and behavioral health disorders to achieve optimal outcomes for children/adolescents in foster care.
  • Ensures adherence to all applicable county, state, and other regulatory bodies.
  • May be required to perform other duties as assigned or perform other duties when necessary.

Position Qualifications:
  • MSW/LMSW/LCSW, MHC/LMHC, MFT/LMFT For MSW permit holders, must obtain a license within one year.
  • MHC permit holders must obtain a license within two years.
  • Experience and skill in providing care coordination, preferably with children and families.
  • Microsoft Office and familiarity with NYS Connections system are a plus as other electronic health records systems.
  • Excellent communication, time management, and organizational skills.
  • Bilingual Spanish/English preferred.
  • Willingness and ability to work from both the Harlem and Bronx office with additional travel to our Brooklyn site.

Salary: $54,000 - $57,000 + Merit Pay + Benefits.
