East Neck Nursing & Rehab Center is looking for FT and PT CNA's for the following shifts:

7am-3pm (MUST Work Sunday, Monday, Friday & Saturday)

Position Summary:

Assist registered professional nurse and licensed practical nurses by performing general routine nursing care and related duties in caring for the personal needs of the residents. Works under the direction and supervision of a licensed nurse, and according to accepted policies and procedures.

Qualifications:

  • Satisfactory completion of the Certified Nursing Assistant Training course.
  • Nurse aide certification that is current, verified via Prometric Online Search and in good standing.
  • Previous nursing home or hospital experience is desirable.
  • Ability to demonstrate a caring, gentle, holistic approach to the geriatric population, as well as those residents with special care needs.
  • Able to receive guidance and supervision in an accepting manner.
  • Able to read, write, and verbalize in English.

Duties and Responsibilities:

  • Answers all call lights or responds to resident’s verbal requests to meet resident’s needs.
  • Makes frequent rounds to all residents to determine and meet resident’s needs and follow their plan of care.
  • Provides and assists with all aspects of personal hygiene, partial bath, bed bath, whirlpool tub bath, shower, shampoo, oral hygiene, shaves, nail care.
  • Provides and assists with all aspects of dressing and undressing, appropriate dresses, pants, shirts, sweaters, undergarments, shoes socks, stockings, hair combing, jewelry, makeup.
  • Identifies and reports clothing needs to the charge nurses: lack of items, repair of torn or ripped items, replacement needs.
  • Meets the toileting needs of the residents including answering toileting requests promptly, assisting on and off the toilet or bedpan, providing a urinal, emptying urinals and bedpans promptly, changing briefs, performing perineal care. Documents bowel movements.
  • Provides skin care by washing, drying, lubricating, and observing for any changes in the skin; reports changes in the skin to the nurse in charge immediately.
  • Assists with meeting the nutritional needs of the residents by: preparing the resident for meals; passing trays, encouraging completion of assisting with and/or feeding the residents, providing adequate hydration, providing nourishment, reporting and documenting consumption.
  • Assists with or transfers residents from bed to chair or to a standing area position.
  • Transports residents via chair or lounge from one area to another.
  • Assists with or ambulates residents as prescribed for specific residents with corresponding documentation.
  • Carries out a bowel and bladder training program as prescribed for specific residents with corresponding documentation.
  • Assists with or performs range of motion as prescribed for specific residents with corresponding documentation.
  • Ensures the use of eyeglasses, hearing aids and dentures, as appropriate to specific residents.
  • Ensures the use of prosthetic devices, assistive devices and adaptive equipment, as appropriate to specific residents.
  • Encourages orientation of the resident to person, time, and place. Participates in Day Room Programs.
  • Communicates with all residents, including those unable to respond appropriately.
  • Demonstrates an understanding of the interests and needs of the geriatric resident and treats the resident with kindness and respect, observing resident rights.
  • Makes beds – unoccupied and occupied – and maintains the cleanliness and neatness of the resident’s unit, bedside stand, dresser and closet.
  • Turns and positions residents as per established schedule.
  • Properly applies and releases restraints as ordered for specific residents and provides the appropriate activity during the release times.
  • Observes the resident’s skin for any indication that a restraint has been improperly applied or not released and reports such changes to the nurse in charge.

  • Observes all skin areas and reports evidence of redness or breakdown to the nurse.
  • Provides a safe environment and employs all precautions to prevent resident, personal, and staff injuries.
  • Provides privacy for the resident at all times by closing cubical curtains and doors; using bed linen and lap robes; knocking on doors to resident rooms, bathrooms, and bathing areas.
  • Collects urine and stool specimens as directed and empties and measures urine for catheter drainage bags.
  • Measures and records intake and output.
  • Initiates conversations with, reads magazines to, plays records/music for residents in the day room during the assigned period of time to the day room.
  • Assists with admission, discharge, transfer, and post mortem care of residents.
  • Demonstrates knowledge of infection control policies and procedures, i.e., hand washing, correct linen handling, precaution and isolation technique.
  • Knows the proper procedure for fire drills and responsibilities in the event of fire or other disaster.
  • Interacts with residents’ families in a cooperative, friendly and helpful manner and refers them to the appropriate source for information concerning the residents care or condition.
  • Documents daily activities of the residents on appropriate forms and / or in EMR.
  • Reports all findings and changes in condition of resident promptly to the nurse in charge.
  • Reports on and off duty or whenever leaving the assigned unit to the nurse in charge.
  • Receives report regarding the resident’s condition and needed care and utilizes assignment sheet to ensure provision of required care.
  • Conducts self in an ethical manner and maintains confidentiality of resident information at all times both within and outside the facility.
  • Maintains work areas (treatment and utility rooms, pantry, linen carts, etc.) in a clean and safe manner.
  • Attends in-service education programs and meetings as scheduled to improve skills and communication.
  • Works cooperatively with other staff, providing care as a team.
  • Demonstrates awareness of all facility policies and procedures and functions of other departments and their interactions with nursing.
  • Reports on duty clean, well groomed, in proper uniforms and completes assignments neatly and accurately.
  • Performs other duties as directed as the need arises to assure optimum care of residents.

Safety and Sanitation:

  • Follow all established safety procedures and precautions when operating equipment.
  • Report equipment malfunctions or breakdowns to the program manager as soon as possible.
  • Report all unsafe/hazardous conditions to the program manager immediately.

Customer Service:

  • Maintains an adequate liaison with families, residents and patients.
  • Demonstrates awareness, respect and concern for internal customers (coworkers) and external customers (i.e. patients, residents, families, physicians, referral sources)

Residents' Rights:

  • Maintains CONFIDENTIALITY of all resident care information to assure resident rights are protected.
  • Assures that the resident's rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are followed.

Attendance:

  • Observes rules, policies and regulations with regard to attendance, punctuality, appearance and professional attitude as established by the facility.

Miscellaneous:

  • Conforms to the HIPAA Compliance Program and applicable facility policies for patient privacy.
  • Conforms to Facility’s Corporate compliance Program to include Code of Conduct and Conflict of Interest.
  • Conforms to the facility’s policies and procedures.

Physical:

  • Able to stand times (2) plus hours without interruptions or breaks.
  • Able to push, pull, move, and or lift a minimum of 50 pounds to a minimum of 4 feet or carry 50 lbs. a minimum of 2 feet.
  • Must be able to perform all patient transfers, (including stand pivot, 2 person, sliding board, Hoyer Lift, non-weight bearing) safety and effectively with or without mechanical devices.
  • Must be able to sit, stand, bend, lift and move intermittently throughout the day.
  • Must be able to assist with the evacuation of residents during emergency situations.
  • Must pass Medical and Physical examination based on job qualifications, requirements and duties/responsibilities.
