MPower of Sedalia, MO and Marshall, MO is seeking to hire a full-time( 40 hours a week)* Case Manager / Service Coordinator* to assist and support the team by providing professional case management services to eligible individuals across all counties. Case Managers / Service Coordinators are the backbone of our company as they work directly with our clients who have developmental disabilities.

Summary of Job Description

On a daily basis, provides professional service coordination services to eligible individuals by performing the following duties in an environment that promotes the individual served:

Applies professional judgment and discretion in determining and securing Medicaid and non-Medicaid habilitative services to ensure both active treatment and maintenance of skills aimed at preventing or reducing out-of-home placement and/or institutionalization of individuals served.

Supports and/or refers individuals served to navigate complex requirements and regulations to receive appropriate services.

*
Manages a caseload of moderate size and complexity, normally involving a wide range of disability areas and age ranges.

*
Managing a caseload may include administrative duties such as tracking, mailings and utilization review.

*
Secures information such as medical, psychological, and social factors contributing to individual’s situation to assist in completing the Individual Support Plan (ISP); secures supplementary information such as employment, medical and/or school records. The ISP is written by the Service Coordinator in conjunction with the individual, family, guardian, providers, etc. according to MPower and Department of Mental Health – Division of Developmental Disabilities (DMH-DDD) guidelines.

Maintains a working knowledge of and locates, refers, arranges, and accesses community resources available for the individual served.

Administers and/or reviews both formal and informal assessments with individuals served; including, but not limited to, the Missouri Critical Adaptive Behaviors Inventory and Vineland for determining eligibility for Medicaid Waiver funding or ongoing eligibility of services. Staff will be trained/certified upon hire, as appropriate.

Develops and maintains records regarding services rendered and prepares reports concerning the individual served and progress toward ISP outcomes.

*
Facilitates team meetings to discuss progress and possible modifications to services in the ISP; completes addendum's as needed.

Makes contact with each individual served as required by MPower policy and DMH-DDD directives.

*
Serves as a liaison between multiple agencies and individuals served to ensure that the individual’s needs are being addressed.

Miscellaneous duties

*
Attends and participates in staff meetings, in-services and staff development training.

*
Utilizes the KRONOS time keeping system to clock in and out while performing work duties and request time off. Maintains at least a 40-hour work week.

Accepts and willingly carries out special assignments or duties, when requested.

*
Projects positive image of MPower and individuals served to the local community.

*
Assures the confidentiality of information regarding individuals served.

*
Respects the rights of all individuals served.

*
Utilizes personal vehicle for the completion of work duties. This includes, but is not limited to, driving to appointments, training's, and meetings while on the clock.

*
Replies to all correspondence and phone messages within 24 hours.

The employee will frequently be required to use the following programs/devices: SET-works, Microsoft Outlook, Microsoft Word, Microsoft Excel, Microsoft TEAMS, computer, KRONOS, Bamboo HR, Employer Advantage portal, and other agency and State of Missouri programs/databases.

Certificates. Licenses, & Registrations

Must have a valid driver's license at time of application

Must demonstrate mastery of specialized case management training as approved by the Missouri DMH-DDD within 90 days of hire.

Education and Experience:

*
MPower prefers one or more years of professional experience as (a) a registered nurse, (b) in social work, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, or speech therapy or a closely related area, or (c) in providing direct care to people with DD

Must meet requirements set forth by CMS (Center for Medicaid Services) and DMH-DD (Department of Mental Health Division of Developmental Disability) requiring Case Managers (Service Coordinators) employed by a provider of Targeted Case Management to have the following:

1. A Registered Nurse license; or

2. A Bachelor’s degree from an accredited college or university

Case Managers employed by a qualified provider on or before June 30, 2018 shall remain qualified.

These requirements are the same as are required for the Missouri state merit position of Case Manager I. Anyone who has worked for the state as a Case Manager I, or who is on the register for Case Manager I is considered to have met the requirements as a TCM Case Manager, also called a Support Coordinator

3. Temperature-controlled climate of an office setting

Due to the nature of the Service Coordinator’s work directly with the individual served and or provider, often times the Service Coordinator finds themselves in the field. MPower provides modern technical equipment in order for the Service Coordinator to be mobile. This may mean working on site, in the individual’s home, at the office or from the convenience of the Service Coordinator’s home. A Service Coordinator has the opportunity when working away from the office to conduct their business in a paperless fashion. Reimbursements and stipends are available to cover additional expenses that a Service Coordinator may incur while in the field.

Pay/Wage Differentials

Wage differentials will be considered and offered when the Service Coordinator case load requires extensive travel or additional administrative duties.
