CASE MANAGER

Reports to: Clinical Director

Summary of Function

The Case Manager is responsible for assessing, developing and providing integrated, community based services with resources and interventions designed to heighten independent living, individual/ family/community functioning through the provision of services to individuals with specific needs.

Organizational Duties and Responsibilities

  • Can support the Home's mission and vision of The Methodist Home as evidenced by compliance with all organizational policies and procedures.
  • Supports and facilitates positive interaction with others as evidenced by: professional maturity, respect for others, a team-centered approach, maintenance of confidential information, and an appreciation of a multicultural workplace.
  • Exhibits effective communication skills including proper use of agency communication systems.
  • Exhibits sound stewardship by effectively managing information resources to achieve the objectives of The Kentucky United Methodist Homes for Children and Youth.
  • Supports all functions that attain and maintain accreditation with regulatory agencies.
  • Participates in appropriate development programs and in services to attain and maintain competency.

Supervisory Duties

  • Encourages innovation, creativity, and goal setting with clients.
  • Promotes team building and teamwork.
  • Is primarily responsible for the overall well-being and support of the clients, assuring that the clients are cared for adequately.
  • Provides on-the-job training for new employees in conjunction with the Youth Services Coordinator, Therapist, Medical Coordinator/Nurse, other assigned Youth Care Workers, and Therapeutic Services Director, to aid in the completion of new employee's orientation packet.
  • Assists Youth Services Coordinator as assigned with regular supervision meetings and performance evaluations of Youth Care Workers.
  • Mentors Youth Care Workers to develop/maintain effective skills necessary to respond to traumatized clients and support a trauma informed care system.
  • Motivates the Youth Counselors to work cooperatively for the good of the program.

Job Specific Duties and Responsibilities

  • Demonstrates professionalism, cooperation, leadership, and a clear understanding of program purpose.
  • Initiates and develops therapeutic relationships with youth in the program. Provide supportive therapeutic services to assist the youth to improve functioning.
  • Assesses housing, inspects youth apartments per policy to assure agency standards as well as all monitoring agencies standards are in compliance. Assists with and works closely with Facilities to serves as liaison with landlords as needed and keeps Therapeutic Services Director apprised.
  • Collaborates with youth and other invested parties in the development and implementation of the youth's Comprehensive Independent Living Plan.
  • Assist in assuring thorough communication with youth, families, and the involved agencies.
  • Consults with the Youth Counselors, Therapist, Youth Services Coordinator, and Therapeutic Services Director regarding youth's progress in the development of independent living skills.
  • Maintains case management responsibilities per policy.
  • Meets a minimum of weekly with youth in the independent living program to provide individual instructions and review progress in the program. Meets daily with youth in IL that are 17 and younger.
  • Makes appropriate referrals to social service agencies to provide collaborative services, education, and training for residents in the areas of independent living.
  • Conduct and document appropriate assessments. Maintain current knowledge of community resources and refer clients as needed.
  • Assists youth in reviewing options for higher education, completing applications and financial aid forms.
  • Ensures that the staff responsible assists the youth with meal planning and grocery shopping per policy.
  • Assists youth in obtaining employment within the community. Provides residents with job skills through job coaching and assessing job performance. Acts as a liaison between KYUMH and youth's employer.
  • Assists youth in completing the statewide mandatory independent living curriculum and communicates directly with the Youth Services Coordinator, Therapist, Residential Services Director and Therapeutic Services Director about the youth's progress.
  • Identify, develop, and document intervention strategies.
  • Provide transportation periodically to assist the youth in keeping scheduled appointments or school attendance.
  • Maintain and document contacts with all youth including a minimum of weekly in-home visitation.
  • Facilitates regular life skills groups with clients.
  • Provide liaison services between KYUMH programs, school personnel, courts, community agencies, employers, and other stake holders.
  • Participates in and transports to identified meetings/hearings as needed, including but not limited to, program meetings, DCBS meetings, school hearings, court hearings.
  • Assures that Therapeutic Services Director is promptly advised of all significant incidents and client progress.
  • Ensures the competent and timely completion of required reports for the Methodist Home and all appropriate outside agencies.
  • Attends professional meetings, workshops, seminars and other training sessions necessary to maintain professional expertise.
  • Maintain regularly scheduled meetings with supervisor.
  • Completes all other duties as assigned by Therapeutic Services Director.

On-Call Responsibilities

  • Remain physically available to respond to crises as dictated by agency policy.
  • Be available to all programs to answer questions and address staff concerns. Make certain that staff is following correct procedures.
  • Be available by phone after hours; be no more than one hour from campus at any point

during on-call.

  • Assure that staff completes Incident Reports as required.
  • If SCM is utilized, debrief all staff involved.
  • Notify Therapeutic Services Director according to procedure.

Job Specifications

  • Must be at least 21 years of age.
  • Must have a valid Kentucky driver's license.
  • Successful completion of a thorough background check.
  • Bachelor's Degree in related field.
  • Maintains required annual training as applicable per education level. Additional training is required and provided for those without a degree or experience.
  • Is trained in CPR and First Aid. Remains current.

Physical Demands

This position requires the employee to frequently engage in speaking on the phone, computer skills, filing, public speaking, and walking up and down stairs. In addition, Case Managers must be able to drive children out of town to meet required contacts or court dates. Case Managers are also required to maintain current training status in ESPI and therefore must be physically able to perform ESPI training.
