Responsibilities:
Case Manager, RN, PRN

The Case Manager is responsible for providing case coordination and functions as a patient/family advocate through initial and ongoing assessment of patient’s needs during the rehabilitation stay, coordination/communication of the patient’s plan of care and identifying and coordinating community resources to meet continuing care needs. The Case Manager will coordinate communication with payer sources and provide updates as necessary to secure ongoing authorization to treat. He/she will participate in the preadmission screening process. He/she shall participate in and demonstrate accountability for quality improvement, overall team coordination (as it applies to patient care issues) and discharge planning.
  • Demonstrates strong working knowledge and skill experience with a variety of diagnostic groups including, but not limited to, the following: orthopedic and arthritis related conditions; neurological groups including CVA, TBI, degenerative neurological diseases, SCI; generalized weakness as a result of medical or surgical conditions including DM, ESRD, amputation, COPD, CHF, and CABG.
  • Demonstrates a working knowledge of normal growth and development, related behaviors, needs, comfort, and range of treatment appropriate to the age (adult, geriatric) and diagnosis of patient services to support the provision of an individualized interdisciplinary plan of care.
  • Meets with the patient and family prior to or at the time of admission to identify needs, 3rd party payer requirements, orients to rehab process and begins discharge planning and completes assessment according to protocol.
  • Educates and instructs patient/family regarding role of Case Management in the rehabilitation program and needs identified during the assessment.
  • Collaborates and communicate with the team and patient and to implement identified discharge plan for the plan of care, adapting the plan of care to the individualized needs/goals of the patient/caregiver assessing opportunities and barriers.
  • Provides orientation to the patient and the family about the services of the program, the outcomes the program and the role of the team members in relationship to the patient.
  • Serves as a patient advocate, acting as a liaison between the patient, hospital, and community resources, discussing discharge options with the patient and/or family to help guide the team on the needs, wishes and capabilities of the patient and the support system.
  • Discusses patient rights with the patient and family members or caregivers in coordination with the Social Worker.
  • Demonstrates knowledge of management of patients with maladaptive behaviors.
  • Plans, organizes, and prioritizes assigned caseload to provide efficient cost-effective care reflective of patient needs, within assigned workday, with consideration for gender/age specific guideline, choosing the most appropriate treatment method to achieve desired goals.
  • Demonstrates ability to effectively review the patient’s chart and communicate/collaborate with multidisciplinary team members regarding patient’s condition.
  • Demonstrates ability to effectively communicate with managed care providers to complete concurrent authorization for patient stay. Assists with pre-authorization as requested.
  • Demonstrates the ability to instruct patients/caregivers regarding issues related to their plan of care and/or discharge plan in a language/style the patient can understand.
  • Completes required documentation of professional involvement and interaction including assessment, progress notes no less than weekly, and other documentation in a timely fashion according to policy.
  • Plans, organizes and prioritizes assigned caseload to provide efficient, cost-effective care reflective of patient needs based on gender/age specific guidelines and completes caseload and documentation within assigned workday according to departmental expectations.
  • Continuing education pursuits are made within scope of discipline and relevant to caseload/patient population.
  • Assists with training, supervision, and coordination of staff efforts with assistants, students and techs.
  • Makes appropriate referrals to Neuro and SW as needed.
  • Performs other duties as required and assigned.
Qualifications:
  • Current RN licensure from the State of Oklahoma required.
  • ADN required; BSN preferred
  • Current BLS required or obtain at time of hire.
  • First Aid certification preferred.
  • 2-4 year’s clinical nursing experience in med/surg or acute rehab setting preferred.
  • Experience in case management preferred.
  • Leadership experience preferred.
  • Basic computer skills required.
