This is a 1099 position.
Case Manager to enhance the quality of client management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full client care cycle. Case manager duties include assessing, and evaluating actions required to meet the clients need. Help seniors gain access to needed medical, social, educational, financial and other necessary services.
Will carry a case load of 20 to 40 clients
QUALIFICATION AND EDUCATION REQUIREMENTS: To be eligible as case manager, an individual must meet the following criteria:
- Bachelor Degree in social work, counseling or related field from an accredited college or university.
- Associate Degree and 5 years experiences.
- One year of full time or equivalent experience working with seniors
- Have knowledge of available resources in the service area for children and or adults with serious mental illness.
- Open for contractual position
- Test and Training 701 B
- Background Check Level 2-
