Summary: The primary purpose of the Case Manager is to work collaboratively to address the case management needs of the women - housing, employment, recovery, health needs.

Job Requirements:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, minimum education, training, licensure, experience, and / or ability required to accomplish this employment expectation.

Bachelor's Degree in Social Work, Psychology or related field. Degree verification required.

  • Familiarity with homelessness, mental health issues, domestic violence, and substance abuse.
  • Computer skills, preferably performing computerized psychological and substance abuse assessments.

Desired Qualifications:

  • Knowledge of the community’s resources and other social service agencies.
  • Bi-lingual.
  • Trained in trauma informed care principals or trauma specific interventions.

Essential Duties and Responsibilities / Core Competencies:

1. Coordinate employment, housing, health, mental health, and recovery services for mothers and their children.

2. Support individual treatment plans.

3. According to treatment plans, provide necessary case management to mothers and their families.

4. Complete admissions paperwork.

5. Provide referrals for families when appropriate and/or necessary.

6. Support families in Aftercare and Outpatient programs.

7. Complete documentation of individual, group and family activities.

8. Maintain complete participant files including weekly sessions, progress notes, treatment plans, assessments and monthly treatment progress summaries.

  • Document all individual interaction.
  • Document group activities.
  • Complete monthly reports and billing as required by grant contracts.

9. Provide group leadership and facilitation.

10. Attend and participate in clinical and staff meetings.

11. Use productive conflict resolution skills to confront all incidents of prejudice.

12. Educate residents on diversity, through role modeling and educational materials.

13. Provide emergency on-call coverage when appropriate or necessary.

14. Support the mission, purpose, values, and objectives of Sheffield Place.

Pay: $40,000.00 - $45,000.00 per year
