RHD's Coastal Wellness is looking for a Full-time Case Manager to join our Team!

The case manager will determine the resources needed to meet the individualized needs of the participant. The case manager will take all necessary actions in assuring the implementation of the service treatment plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps, and recommending strategies.

About the Program:

RHD Coastal Wellness offers a variety of flexible services and subsidized housing to assist mental
health consumers in their transitions from a hospital setting to the communities of Monmouth and
Ocean Counties. This housing model provides intensive in-house support based on individual needs.
By providing consumer choice and a more focused system of support, participants can achieve
stability within the local community as an alternative to residential group care, gaining fuller access to
community resources and a greater measure of independence.

Some Responsibilities Include:
  • Act as part of a multidisciplinary team designed to provide services from a holistic and Values-based perspective.
  • Assist in the development of recovery plans and reviews in collaboration with the consumer, natural supports, and consented stakeholders.
  • Provide community-based services to consumers focusing on education, connection with resources, community interaction, and achievement of recovery plan goals.
  • Complete all assessments and documentation as required by RHD, funder, licensing/ accreditation entities.
  • Develop and maintain relationships with community agencies in order to assist a person in connecting with all available resources.

Pay Rate: $20/hour.

Education & Experience:
  • A Bachelor's degree in human services or related fields.
  • 2 years of experience in a similar field.
Qualifications:
  • Intermediate computer skills.
  • Preferably one year of Case Management experience.
  • Valid driver’s license.
  • Must pass Federal and State criminal background checks.
