Nature of Work

The Opioid Crisis Response Program (OCR) was established as an intervention strategy for reducing incidents of opioid overdose within Coconino County. The OCR program provides individuals with peer support, harm reduction education, linkages to treatment and supportive wraparound services. The Case Manager Apprentice will work closely with the team to provide compassionate ongoing support, case management and care coordination for clients throughout their recovery journey. The role requires strong communication and organizational skills and the ability to foster a non-judgmental environment for people who use drugs.

Under supervision performs work of moderate difficulty in assisting and shadowing in related field; performs related work as assigned.

After incremental training in the apprentice position, employee will be able to demonstrate the ability to assist and work independently, autonomously, and consistently perform job duties.

Please Note: This is a limited appointment position funded through the HHS Apprenticeship Program and is expected to end no later than June 30, 2024.

Typical Duties

  • Works closely with Recovery & Resiliency (R&R) staff to progressively train in standard professional case management across several health and human services programs. Training will include learning to assist in case management and providing support and services to clients.
  • Become proficient in linkages to care.
  • Become proficient in assessing client service needs, including behavioral health support (OUD, SUD, etc.)
  • Under supervision of R&R program staff, will become proficient in maintaining updated client records with plans, notes, appropriate forms, or related information
  • Under supervision of R&R program staff, become proficient coordinate services with program staff and community-based organizations, public agencies, coalitions, schools, justice agencies and other appropriate facilities to provide coordination and facilitate referrals for individuals
  • Under supervision of R&R program staff may assist clients in achieving goals relating to housing, employment, education, relationships, etc.
  • Under supervision of R&R program staff, will learn to identify or contact members of vulnerable populations.
  • Under supervision of R&R staff may assist on conducting home visits and regular check-ins with clients to provide outreach and engagement
  • Become proficient in referring community members to needed health and social services or other identified needs; assists community members in navigating systems of care
  • Become proficient in providing feedback to Health and Human Services programs regarding improving service accessibility or acceptability
  • Become proficient in developing and maintaining positive relationships with others, including program participants and other collaborating partner staff
  • Under supervision of R&R program staff, will become proficient in contacting clients in person, by phone, or in writing to ensure they have completed required or recommended actions
  • Under supervision of R&R program staff conduct outreach and follow up for program participants
  • Under supervision of R&R program staff, will become proficient in advocating for individuals or community health needs with government agencies or health services providers
  • Become proficient in attending community meetings/outreach and collaborative meetings to build relationships with community members, and provides resources to care and services
  • Under supervision of R&R program staff, will distribute flyers, brochures, or other informational or educational documents to educate/inform members of a vulnerable community of applicable resources
  • Performs related duties
  • Utilizes recovery-oriented, trauma-informed, and person-first language in all interactions and demonstrates excellence in compassionate customer service
  • Adheres to workplace procedures as well as health and safety protocols
  • Attends meetings and offer suggestions for improvement
  • Compiles reports and summaries of work
  • Updates, checks, and completes records, forms and documents
  • Verifies discrepancies and makes corrections
  • Processes documents to update and maintain files and records
  • Researches records and disburses information
  • Processes and distributes confidential and sensitive documents
  • May assist in transporting clients
  • Maintains an electronic filing system as appropriate for the program
  • Composes, formats, and types routine correspondence, reports, and other documents
  • Operates and maintains a variety of office equipment
  • May perform data entry
Essential functions of this position include: driving; sitting for computer entry, typing, answering telephones, etc; working with and around other staff, administration, and citizens, etc; dealing with interruptions; repetitive motion (hand / wrist) for computer and phone work; vision acuity (near / far); hearing and speech for ordinary conversation; touch and finger dexterity for using telephones and computers.

Minimum Qualifications

High School Diploma or G.E.D., a desire to work within specific program and one year of previous work experience in any industry OR, any equivalent combination of education, training and experience which demonstrate the ability to perform the duties of the position.

The ideal candidate will be committed to serving diverse communities, have experience or interest in serving individuals with behavioral health needs, experience or interest in case management, have an awareness of the role of public health, will be ready to learn and grow in this position, with skills and experience in engaging with individuals respectfully.

Preferred Qualifications: Experience working with individuals who are justice involved or deal with substance use disorder.


This position is non-exempt from overtime.
