Projects for Assistance in Transition from Homelessness
(PATH): Project for Assistance in Transition from Homelessness (PATH) is an intensive case management service which provides mobile outreach services to the psychiatrically disabled homeless living in the community. This program is designed to reach out to consumers who have not accepted traditional center-based mental health services. This service includes: supportive services, advocacy, housing assistance, mental health services, substance abuse services, and transportation.

  • Position Summary: The Care Manager works in collaboration and continuous partnership with clients, community services, and providers to ensure the client's care plan is effectively implemented and adequately addresses the client's needs. The role will connect clients to relevant community resources including substance abuse treatment, housing, employment, mental health services and primary care.
  • Qualifications: The minimum requirement is Bachelor's degree in psychology, social work or a related field, plus 2 years of relevant experience. Must have and maintain a valid driver's license with an acceptable driving record

Integrated Case Management Services
: Integrated Case Management Services (ICMS) engages individuals at the point of hospitalization and participates in the discharge planning process to assist with the transition from hospital to community. Case management provides post-discharge follow-up to support the client with community integration. This is done through direct in-home contact and linkage to community resources. The client is linked to resources that support the ability to remain stable within the community.

  • Position Summary: The Case Manager works in collaboration and continuous partnership with clients, community services, and providers, and the clinician team to ensure the client's care plan is effectively implemented and adequately addresses the client's needs. Involuntary Outpatient Commitment
: Involuntary Outpatient Commitment (IOC) serves individuals diagnosed with serious, persistent mental illness who, due to their mental illness are dangerous to themselves, others or property in the reasonably foreseeable future. These individuals have also demonstrated a history of positive treatment responses in an outpatient setting. IOC is an opportunity to effect change before an individual reaches a crisis point which results in an inpatient hospitalization.

  • Position Summary: The IOC Care Manager provides individualized, collaborative and flexible outreach services designed to engage, support, and integrate consumers with serious mental illness into the community of their choice and to facilitate their use of available resources and supports in order to maximize their independence. Services are designed to assist consumers in their recovery by helping them gain access to needed mental health, medical, social, educational, vocational, housing and other services and resources.

Adolescent Case Management Services


Life Link Homes
: Life Link Homes offers permanent supportive housing for youth who are homeless or aging out of the child welfare system. The program provides life skills, employment counseling, and therapy to ensure a successful transition to self-sufficient living and to prevent long-term dependency on social services. Young adults who comply with their lease requirements may remain until they determine they have the skills and self-confidence to live fully independently.

  • Position Summary: The Adolescent Community Resource Specialist will act as an agency leader in marketing the Adolescent Supportive Housing Programs, and will represent the agency in the community by attending and presenting at meetings. Serve as initial contact for Life Link Homes and Steps Towards Independence Adolescent Supportive Housing Program applicants and referral sources. Process all program applications; assist applicants through housing process; act as an active participant in the interview process. Collaborate with Independent Living Coordinators in preparing young adults for meaningful employment. Ensures the safety, stability and well-being of youth and effectively work with and identify services/supports for youth who may be facing a range of co-occurring challenges, including mental health, domestic violence, substance use, economic challenges, lack of support, and other needs. Liaison with vocational and educational community partners to help youth develop career plans and obstacles to employment.

Mobile Response and Stabilization Services
: Mobile Response provides immediate intervention services for children and adolescents experiencing escalated emotional and behavioral health issues and provides case management services for up to 8 weeks to the youth and family to prevent further problems in functioning and/or disruption of their living environment.

Connect to Home
: Connect to Home provides permanent supportive housing for youth and/or parenting youth ages 18 to 21 who are at risk of homelessness and are currently or were previously involved with the Division of Child Protection & Permanency. Staff provide youth with support services to enhance life skills, education, employment, community supports, and stability.

  • Position Summary: Provide case management, outreach, engagement, and support to youth residing in supportive housing units in Burlington County. Ensures the safety, stability and well-being of youth and effectively work with and identify services for youth who may be facing a range of co-occurring challenges, including mental health, domestic violence, substance use, economic challenges, lack of support, and other needs; Liaison with vocational and educational community partners to help youth develop career plans and obstacles to employment. Provide individual support and counseling and assist with crisis management and medication monitoring. Help youth secure public benefits, and develop recovery strategies. Train and build the competencies and skills of youth.

Family Case Management Services


Keeping Families Together
: Keeping Families Together provides both short and long-term supportive housing for high risk families who have a current or ongoing history of homelessness, with children at risk of removal or soon to be reunified. The program provides housing resource specialty, case management, therapy and support groups to families who receive either a program-based or rental assistant voucher.

  • Position Summary: The case manager provides support services and case management to families with a history of homelessness or housing instability. Assist parents to make a successful transition to self-sufficient living and improved individual and family stability. Provide coaching and guidance on life skills such as home management, nutrition, budgeting, parenting, education/vocation training, advocacy skills. Co-facilitate groups for parents, children and families. Link families to both formal and informal community resources. Work collaboratively with your KFT peers, DCP&P and community service providers.

Visitation Services
: Our Visitation Services program promotes and maintains the family-child bond by facilitating weekly visitation with the children in foster care and their parents, relatives, and siblings or non-custodial parent. The goals of the programs are to improve parenting and achieve permanency.

  • Position Summary: The Visitation Counselor helps maintain the parent-child bond by providing visits for children in foster care and their parents. Coordinate visits between parents and resource parents and transport children to and from visits. Provide visit planning, coaching techniques and debriefing; helping to improve parenting skills and interactions so reunification or permanency can occur. Facilitate meaningful visits; helping parents meet the physical, emotional and developmental needs of their children, ensure the safety of children and model appropriate interactions between parents and children, work collaboratively with peers, DCP&P and other community providers.

Healthy Families
: The Healthy Families program provides in-home education and supportive services to new and expectant parents. Home visitors work to link parents to existing social service and health care resources, and promote positive parenting and healthy child growth and development.

  • Position Summary: The Family Support Worker will outreach families referred through Central Intake or a Service Provider. FSW will use a standardized screening tool to determine eligibility for home visitation services. Interview and conduct the Parent Survey with parents to identify strengths and risk factors to support the family throughout home visitation services. Assist in strengthening the parent-child relationship. Assist parents in improving their skills to optimize the home environment. Assist in increasing the family support system and increase the family's ability to problem solve and assume the role of advocating for themselves and their children. Responsible for assisting the family in establishing goals and a plan for the accomplishment of those goals.
  • Qualifications: Minimum of a High School Diploma or GED equivalent with two years of experience in working with children (0-3 years of age) and families or, knowledge of normal child growth/development and parent-child relationships.
