The Case Management Assistant assists the inpatient case management staff by completing tasks necessary to facilitate the smooth transition of patients to agencies or services for post-hospital care.

Education Requirements

High school diploma or equivalent required; Associate or Bachelor’s degree in medical or human services field preferred.

Experience Requirements

• A minimum of two years of previous experience in a hospital or healthcare setting; prior experience in a case management related are preferred

Skill Requirements

• Knowledge of medical terminology
• Self-starter, ability to work with minimal supervision
• Able to effectively multi-task
• Proficient with Microsoft Office, specifically Excel and Word.

• Excellent communication skills, telephone etiquette and interpersonal skills.
• Ability to set priorities and handle multiple tasks with careful attention to detail.
• Able to identify and report quality-of-care and risk management issues in a timely fashion.
• Demonstrates competency in the operation and maintenance of basic office equipment
