Community Horizons is growing and hiring a Support Broker to cover Wood, Waushara, and Green Lake Counties.

What is a role of a Support Broker?

Support Brokers are expected to provide services to individuals enrolled in self-directed programs to have the opportunity to lead a self-determined life and reach their goals. In this role, you will work directly with individuals in their homes on the recruitment process of their caregivers which will be carried out through the following tasks.

· Participate in new referral/home visits based on members' needs and availability

· Writing creative job posts and utilizing platforms to seek applicants

· Participate in phone screens and interview applicants for each individual position

· Assist with the onboarding process such as new hire paperwork

· Document services and supports provided otherwise known as case notes

This position is a semi-remote position which means you work from a home office with 75% travel expected within the assigned region. This is a flexible schedule position however you are expected to be available during normal business hours and possibly some evenings depending on member availability.

Qualifications.

• A minimum of 5 years of experience working with the population of frail elderly and/or individuals with disabilities
• Attentiveness/Detail Oriented
• Great communication skills both written and verbal
• Ability to manage a caseload effectively giving individual attention to each member
• Knowledge and experience in recruiting within the healthcare field required
• A clean background check required
