Position Summary

A Care Program Coordinator provides support and assistance in the identification, development and delivery of educational Care Programs or activities. This person works under direct and frequent supervision by the care programs management while performing routine duties using established procedures with detailed instructions.

  • Assist with planning and coordination of programs and their activities
  • Monitor implementation of program policies and practices to promote a unified policy compliance
  • Work to keep programs on schedule and functioning smoothly
  • Support program growth and development as necessary
  • Coordinate program communications
  • Assist with audit team results, identify gaps in training
  • Recommend innovative plans for design or quality improvement of unit programs/procedures
  • May lead internal project team
  • May participate in or lead committees
  • Periodic reporting to Nursing Department of assigned tasks
  • Serve as a communication liaison between care coach/nurse educator staff and administration
  • Create and maintain program records, reports, presentations, and proposals
  • Facilitate positive relations between the program team and other departments within the organization, and all other involved parties
  • Support Candidate Interview Process as organized by Nursing leadership
  • Performs miscellaneous job-related duties as assigned
  • Availability to occasionally work or meet in the evenings and/or on weekends
  • Flexible with traveling to different Sanitas Locations within the state


Supervisory Responsibilities

This position has no supervisory responsibilities.


Required Education and Experience

  • Associate degree or higher or an equivalent combination of education and experience in the healthcare field
  • Proficient in EMR, Excel, Word, and Power Point
  • Communicates effectively with interdisciplinary teams
  • Excellent organization, leadership, interpersonal and clinical skills
  • Recordkeeping
  • Self-motivate, flexible and creative
  • Multitask
  • Bilingual (English/Spanish)

Preferred Education and Experience

  • 1+ year in primary and urgent care setting
  • ECW experience
