Day Shift; 8:00am - 5:00pm

Exempt; Pay Grade: CL

Position Summary:

The Care Manager is an integral member of the primary care team, tasked with leading efforts to engage patients and coordinating care for high risk and high need patients with complex health care and social needs. The Care Manager, working alongside the primary care provider, proactively anticipates and coordinates care services across multiple providers and care settings and provides comprehensive self-management and health education for targeted member populations with chronic disease conditions or high cost specialty care. She/he demonstrates knowledge of complex care management core competencies, disease prognosis and outcomes, and is responsible for ensuring member goals are achieved within a specified time period based upon individual needs, reasonable expectations and well-documented outcomes.

Education:

  • Bachelor’s Degree from an accredited school of nursing/related field; or RN currently enrolled in a bachelors program with a documented plan for completion.
  • Equivalent experience may be substituted for a degree.
  • Bachelors degree in another healthcare related field such as occupational therapy, social work, psychology, or counseling with professional licensure or registration in a clinical field where required also accepted.

Required Licensure, Certification, Etc.:

  • See Education. Certification in Care Management (CCM) preferred.

Work Experience:

  • Minimum five (5) years clinical practice. Three (3) years of care management experience. Managed Care experience preferred. Current registered nurse licensure in the state of Maryland. Certification in case management (CCM) preferred.

Knowledge/Skills:

Knowledge: Demonstrated clinical expertise. Standards of medical practice. Knowledge of health care and managed care delivery systems. Insurance benefit structures and related legal/ medical issues. Quality improvement processes. Community resources. Clinical, satisfaction and financial outcomes.

Skills: Ability to work in a dynamic, team-oriented environment and demonstrate positive relationship building. Effective verbal/ written communication. Ability to affect change. Ability to perform critical analysis, plan and organize effectively. Ability to promote member/family autonomy. Business acumen so that health benefits and member/provider satisfaction are maximized while solidifying the program’s viability through cost effective intervention.
