We need a professional who shares our philosophy that “home is the safest place”. You will coordinate with clients and families to create a plan and establish a team of caregivers to implement a personalized approach to achieving the goals of that plan. Our clients need someone to meet with them, hear their concerns and act as liaison between client and office. These clients need a resource who can facilitate client education, empowerment, and quality of life. They need an advocate that can provide recommendations and referrals as needed. They need You!!

Primary Responsibilities.

· Client Assessments and Care Plans; develop/update/change as needed

· Supervise Care; assist in coordinating care team/perform in home visits/caregiver training

· Client Records; maintain and export to outside agencies as necessary

Minimum Qualifications.

· Experience in home care, health care of related field preferred but not required

· Must be legally permitted to work in the United States

· Reliable means of transportation with current insurance and valid license

· Ability to interact effectively with a diverse audience including clients, families, care providers and other professionals

Job type

· This is a part-time 25-30 hours/week position with flexible schedule and potential to grow with company
