GENERAL SUMMARY:

Provide support and guidance to individuals living with dementia, care partners and families on issues related to care giving and dementia in the early stages. Assist with the management and delivery of all early-stage programs and services, including Memory Club, Grad Club, Alumni Club and the Early Memory Loss Conferences.

Essential Job Functions:

  • Provide telephone or in-person screening and assessment of client needs, develop care plans for families, provide referrals to services and conduct follow-up regularly with a focus on issues that stem from early-stage dementia, ensuring adherence to program protocols.
  • Provide care counseling services and feedback on client status for early-stage referrals from health plans and the ALZDirect Connect Program, as well as through the AlzLA Helpline, in a timely manner.
  • Consult with care partners, families of people living with dementia, and professional caregivers about appropriate early-stage dementia care; providing on-going care counseling to care partners for as long as needed.
  • Educate care partners on dementia-related issues with a focus on issues that stem from early-stage dementia.
  • Facilitate early-state groups, including Memory Club, Grad Club and Alumni Club, as well as all other early-stage groups.
  • Provide input toward curriculum and program development for early-stage groups and services.
  • Attend quality assurance meetings and trainings.
  • Provide support for planning of Early Memory Loss Conference.
  • Participate in meeting the goals of the grants that support the services provided by the Early-Stage program.
  • Provide care counseling and crisis intervention as the care counselor of the day on a rotating basis.
  • Establish, develop, and nurture relationships with community partners through networking.
  • Provide direct supervision of MSW student interns as needed.
  • Enter client data, contact information, and consultation notes into the Penelope database in a timely manner, updating as needed.

Qualifications, Skills, and Experience Required

  • Master’s level, licensed or license-eligible psychologist, social worker, nurse, or marriage & family therapist preferred
  • Three to Five (3-5) years’ experience in the aging or dementia care field
  • Excellent written and oral communication skills
  • Computer literate
  • Fluent in Spanish, written and verbal.
  • At least one (1) year of experience focused on Early-Stage Alzheimer's disease
  • Knowledge of community resources
  • Knowledge of database software computer programs
  • Personable, able to work with people with dementia, caregivers and professionals
  • Ability to work with diverse volunteers and staff
  • Well organized, self-starter
