HIRING CARE COORDINATOR

Position: Care Coordinator

Locations: Santa Fe Springs, South Gate, Long Beach

Salary: $21.00 - $28.00 (hourly) Depending on education, experience, skillset and hiring site.

Summary: Under the direct supervision of the Program Manager, the Care Coordinator will facilitate patient care by assessing with patient needs, evaluating treatment options, creating treatment plans, coordinating care, and assist with locating and securing housing needs.

Essential Job Functions:

  • Complete intake and assessment process for assigned patients
  • Provide patients with treatment and therapy schedule
  • Document all services into EHR within required turnaround time
  • Collaborate with Clinical Team for Case Conferencing
  • Complete documentation for authorization and submit to QA Specialist
  • Work with Cohorts on admissions/transitions and step downs
  • Follow and provide consistent communications with policy and procedures
  • Coordinates referrals and linkages to housing, mental health, substance abuse, medical, and various social service providers as needed for the individual.
  • Develop a comprehensive housing procurement, financial and self-sufficiency case management plan with clients
  • Assist in locating and securing housing appropriate to their needs
  • Conduct screenings, and assessments to provide linkage to care
  • Monitor cases by verifying attendance, observing, evaluating treatments and response
  • Maintains records by reviewing case notes, logging events and progress;
  • Enter acquired documentation into LA County HMIS system as necessary
  • Provides monitoring of service providers including routine follow-up with the individuals service providers to assess provision of services per the person centered plan
  • Attends scheduled team meetings, community and hospital-based referral and/or discharge planning meetings
  • Provide on call crisis management support
  • Other duties as assigned by direct supervisor

Qualifications:

  • Minimum two years of case management in treatment modality;
  • Minimum two years of experience in the direct provision of counseling, education, case management human services agency, preferably with substance abuse, mental health, or public health activities;
  • Minimum one year of securing housing in case management;
  • Must be able to pass a criminal history background screening and receive clearance from the County of Los Angeles Probation Department; as well as maintain a yearly CPR Certification, First Aide Certification, physical exam, and Tuberculosis test results;
  • Must be willing or currently registered as an AOD Counselor and will maintain requirements;
  • Valid California Driver’s License and proof of personal auto insurance is required.

Education:

  • Associates Degree in human services or in related field or higher preferred;

Must be able to pass the following pre-employment screenings:

  • Background check, including criminal history (LA CADA considers all applicants equally and will take into account each applicant’s individual life circumstances).
  • Drug and health screen, including negative TB test
  • Employment and education verification
