Job Summary
:
The Behavioral Health Therapist is responsible for providing therapeutic care and services to the participants of the Huron Valley PACE program. Works under the direct supervision of the Behavior Health Manager and indirect supervision of the Medical Director or designee but is independent in the application of job responsibilities. The primary responsibility is talk therapy. Provides care through a variety of modalities, including individual and group therapy. Works independently to assess the needs of, coordinate, and deliver behavioral health services to participants and families/caregivers.

Duties will include provide therapeutic interventions to participants. This service may be rendered at HV PACE or the participants home. Provide interventions to participants living with dementia and exhibiting behaviors, by conducting DICE Case Studies. Evaluate outcomes/progress toward achieving goals of the care plan. Participate in IDT meetings.

Skills and Knowledge


Must be able to read, write, speak and understand the English language and follow written and oral instructions.
Must display patience, tact, a positive disposition, and enthusiasm, as well as be willing to work with a variety of participants with varying cognitive levels.
Knowledge of the principles of human behavior, individual and group therapy techniques (preferably CBT and DBT), short-term interventions, substance abuse assessment and treatment, and relevant regulations.
Individual therapy skills for geriatric participants, analytical thinking and problem-solving abilities.
Abides by all established Huron Valley PACE policies, rules, and regulations.
Only act within the scope of his or her authority to practice.
Meet a standardized set of competencies for the specific position description established by Huron Valley PACE and approved by CMS before working independently.

Required Qualifications
1. Master's Degree from an accredited School of Social Work or equivalent if an LPC or LLPC
2. Minimum of two (2) years experience providing therapy services
3. Minimum of one (1) year working with the frail and elderly
4. Current Michigan Driver License and reliable transportation
5. CPR Certification (or willing to obtain)
