Behavioral Health Licensed Social Worker-Madison County Job at North Alabama Community Care
This salaried position comes with a sign-on bonus up to $5,000, depending upon commitment. Eligible for every other Friday off after completing the 90-day probationary period and meeting productivity goals.
Care Coordination company seeking a behavioral health licensed social worker for Madison County. Hours will be Monday-Friday (8AM-5PM).
Summary of Qualifications:
- Must have a Bachelor of Social Work degree and licensed in the state of Alabama.
- Must have a minimum of two (2) years' experience in a behavioral health setting including community mental health, inpatient setting, or other related settings.
- Must have valid driver's license.
Primary Responsibilities:
Complete health Risk and Psychosocial Assessment and develop care plans. The Behavioral Health Social Worker will proactively identify or receive referrals for Medicaid's EIs who are likely to need NACC's Behavioral Healthcare or Substance Abuse Care Coordination services. Home visits are required.
