The Case Manager is intimately involved in behavioral health patient care coordination from admission to discharge. Duties include but are not limited to admission referrals, discharge planning and implementation, and care coordination. The practice of this professional includes maintaining compliance with the Centers for Medicare and Medicaid and DNV regulations, biopsychosocial assessment, proactive continued care planning, crisis intervention, counseling, evaluation of mental health issues, and interdisciplinary collaboration. As well as follow-up post discharge to ensure warm hand-off to next level of care.

Education Requirements

Master's or Bachelor’s degree in Social Work, or registered nurse with Bachelor’s degree in Nursing.

Experience Requirements

Three years’ experience providing behavioral health services. Utilization review and/or payer authorization experience preferred.

Skill Requirements

The case manager must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. This individual must demonstrate knowledge of the principles of growth and development over a life span and must be able to assimilate and assess data about the patient's medical and psychosocial condition.
