Job Summary:
The Behavioral Health Case Manager is responsible for coordinating care with all Clinical Providers in a community health setting in support of the integrated care model. Case Manager will provide direct clinical case management services to individuals of all ages who have, or are at risk of developing, chronic disease, mental health and/or substance abuse issues. Services include, but are not limited to, developing plans for access to services, acting as a liaison with client, families, programs, and service providers; continuous monitoring of client service needs; advocating; and consulting/educating clients, families and community. May require travel to BRMC satellite locations and external sites to provide case management services as needed. The Behavioral Health Case Manager must demonstrate a sincere dedication and loyalty to the mission, vision, and core values of Blue Ridge Medical Center.
