CNA Job Description:

ESSENTIAL DUTIES:

1. Give bed baths, tub baths, showers, shaves and shampoos as assigned by the charge nurse.

2. Provide AM and PM care as prescribed by the facility.

3. Assist residents with dressing, undressing and all activities of daily living, including addressing psycho-social needs.

4. Serve and remove meal trays, remove trays, feed residents, passes nourishments and provide fresh ice water for each resident.

5. Make resident beds, keep the residents unit neat and clean. Also perform assigned duties related to the maintenance of resident health, comfort and safety.

6. Provide and/or assist with bedpans, urinals and bathroom requirements.

7. Take, monitor and record vital signs and weights.

8. Record necessary information on designated forms, e.g. FAR HS, snacks.

9. Assist with living, turning, transferring and positioning residents.

10. Assist with admissions, transfers and discharges.

11. Answer Call lights promptly.

12. Transport residents to and from activities and dining room.

13. Inform charge nurse of any changes in a resident’s condition.

14. Complete all assigned duties in a competent, professional, timely and satisfactory manner.

Minimum Job Qualifications:

Education: High school diploma, GED or equivalent. Completed training at accredited nurse assistant training facility.

Experience: None

Licensure: Nurse Assistant Certification in the State of Michigan

Skills & Abilities:

Ability to maintain twelve (12) in-service hours annually.
Knowledge of Resident Rights.
Ability to utilize Universal Precautions at all times.
Basic knowledge of computer.

LPN Job Description:

ESSENTIAL DUTIES:

  • Uses a systematic approach to collect both physiological and psychological data of the patient/resident.
  • Analyzes the assessment data using clinical judgment to determine appropriate nursing diagnoses.
  • Identifies measurable and achievable goals for each patient/resident using information discovered with assessment and diagnoses to develop a nursing plan of care.
  • Implements the plan and then continuously re-evaluates the patient/resident status and nursing care to make any necessary modifications to provide the most effective plan of care.
  • Communicates documentation in the patient/resident record in a timely, accurate and concise manner. Maintains competency in EMR.
  • Communicates to promote collegiality and to contribute to the professional development of peers and colleagues.
  • Collaborates with patient/resident, family and colleagues through effective communication to achieve positive patient/resident outcomes. Communicates patient/resident outcomes and changes to multidisciplinary team.
  • Assigns appropriate tasks to unlicensed staff. The nurse also provides follow-up ensuring that the tasks were completed correctly and in a timely fashion.
  • Demonstrates individual ownership and accountability but also is a great team player. Motivates fellow employees to exceed patient/resident care standards.
  • Participates in quality improvement initiatives and utilizes findings to improve nursing practice.
  • Administers and documents medication in accordance with established standards, making certain that the Medication Administration Record (MAR) is complete. Ensures that medications are administered in a two (2) hour time frame and that all patients/residents have received their appropriate medication.
  • Documents in the patient/resident record as appropriate including nursing interventions, nursing notes and all other pertinent problems patient/resident may be experiencing.
  • Makes rounds on the unit every 2 hours or more as needed and instructs CNA’s on patient/resident care issues.
  • Stays up to date with current facility initiatives and incorporates evidence based practice into patient/resident care.
  • Provides support to peers, by sharing knowledge and teaching in a professional caring manner.
  • Provides patients/residents and families with appropriate education materials and information pertinent to the individual’s needs.
  • Demonstrates leadership in the professional practice setting.
  • Integrate evidence based practice and research findings into practice.
