Open Door is seeking a Associate Director, Wellness & Nutrition to work in our Ossining, NY Headquarters. This role is responsible for the overall management, oversight, and coordination of wellness (group fitness, food, and health education) and clinical nutrition programs. The Associate Director ensures that all services are provided according to the patient-centered medical home health care delivery model, with an emphasis on patient-focused, team-based care. Under the purview of the Associate Director is staff management, oversight of daily program operations, and collaboration and coordination with internal and external stakeholders. The Associate Director is responsible for the achievement of productivity and quality goals for wellness and nutrition services.


DUTIES AND RESPONSIBILITIES

STAFF MANAGEMENT

  • Supervises program staff (Wellness Coordinators, Senior Wellness Coordinators, Wellness Specialists, Registered Dietitians, AmeriCorps members) and mentors them in their growth and development
  • Manages new staff recruitment activities including collaborating with Human Resources and interviewing candidates for vacant positions
  • Oversees the orientation and training of new staff; ensures that onboarding prepares new staff to achieve 30-60-90-day goals and successfully complete competency assessments
  • Conducts annual competency assessments and performance evaluations
  • Coordinates with Human Resources regarding personnel matters including the implementation of performance improvement plans if needed
  • Oversees the ongoing training of staff to ensure that staff are knowledgeable and competent in areas required for their roles
  • Facilitates regular team meetings
  • Ensures that staff performs duties in accordance with established Open Door policies and procedures, safety, environmental and infection control standards

PROGRAM MANAGEMENT

  • Develops SMART goals for each program in collaboration with appropriate staff; communicates goals with staff, monitors and ensures goal achievement
  • Oversees productivity and quality of the work of all lines of service; implements interventions to address any identified concerns
  • Implements Dramatic Performance Improvement workflow techniques within the clinical nutrition program
  • Implements standardized workflows and adequate program availability and coverage across all sites
  • Monitors clinical nutrition billing and addresses billing issues as they arise
  • Ensures that all progress notes and other documentation is completed following policies and procedures/guidelines
  • Ensures that all relevant policies/procedures and guidelines are adhered to (e.g., Federal, New York State, Open Door, curricula)
  • Addresses and resolves patient complaints or concerns, following the Open Door incident report workflow
  • Completes accurate and timely reporting and data tracking, entry, and analysis activities for all programs
  • Implements and participates in quality/performance improvement activities
  • Conducts grant management activities including application writing, data and narrative report writing, implementing programs to achieve deliverables, and coordinating site visits
  • Completes instructor training to obtain certification in approved chronic disease prevention and management, nutrition, and other wellness-related educational programs (e.g., National Diabetes Prevention Program, Chronic Disease Self-Management Program, Diabetes Self-Management Program, Self-Monitoring Blood Pressure Program and Cooking Matters) and teaches classes as needed
  • If certified as personal trainer or group fitness instructor, teaches classes as needed
  • Ensures that wellness facilities are neat/organized, clean and safe

COLLABORATION AND COORDINATION

  • Serves as the liaison between the wellness and nutrition programs and internal/external stakeholders, including representation at meetings/workgroups
  • Collaborates and communicates with members of other departments to ensure that services are implemented collaboratively and that staff in other departments are aware of the wellness and nutrition services and utilize them effectively

COMMUNICATION

  • Informs the Senior Director, Community Health of any problems that may affect quality and standards of patient services
  • Fosters good communication within Open Door, and between Open Door and the community by returning telephone calls and emails in a timely manner
  • Demonstrates ability to function in a culturally sensitive manner within Open Door’s philosophy and mission
  • Actively promotes planned care concepts within Open Door

PLANNING

  • Attends staff and other organizational meetings, as designated, to ascertain Open Door’s short- and long-range plans, programs, and goals within areas of responsibility
  • Participates in staff development programs, educational activities, seminars, and training opportunities

COMPLIANCE MANAGEMENT

  • Maintains patient confidentiality at all times per Open Door policies and procedures
  • Adheres to National Patient Safety standards as defined by the Joint Commission on Accreditation of Healthcare Organization and the Open Door Family Medical Centers
  • Prepares and maintains documentation in compliance with Joint Commission standards and applicable city, state, and federal laws
  • Meets annual mandatory training requirements

QUALIFICATIONS

EDUCATION

  • Bachelor's degree with course work approved by the Academy of Nutrition and Dietetics' Accreditation Council for Education in Nutrition and Dietetics (ACEND) plus Registered Dietitian (RD)/Registered Dietitian Nutritionist (RDN) plus Certification as a Certified Dietitian-Nutritionist (CDN) in New York State

  • Master’s Degree in Nutrition/Dietetics

  • Master’s Degree in Public Health or another related field with relevant experience
  • 5+ years of progressive experience in program administration and personnel management, preferably within health care settings

  • Fluent in English and Spanish is preferred

  • Strong interpersonal skills including excellent active listening and clear oral communication skills
  • Highly organized and self-directed, able to multi-task
  • Comfortable working in a fast-paced, high accountability environment

  • Strong writing and reading skills
  • Able to understand and follow detailed instructions

EXPERIENCE

  • Staff management experience required
  • Program management experience required
  • Grant management experience preferred
  • Experience working within health care organizations required
  • Knowledge of/experience in wellness and/or clinical nutrition/dietetics preferred
  • Experience with electronic medical records is preferred

  • Must have valid New York State Driver License and daily access to car
