Job Description
Responsible for all functions in the Anti-Coagulation Clinic (ACC) under the direct supervision of the ACC pharmacist. Assists with daily patient flow and clinic activities. Performs nursing assessment, planning, implementation and documentation of specific Anti-Coagulation clinic patients based on their needs for treatment and education. Performs nurse driven patient assessments. Employees in this job must be competent to provide care to the following age groups: young adult 16-30 years, middle age 30 - 60 years, elderly 60+ years.
Job Duties
Patient Care Management - Provides direct care under the direction of the anti-coagulation clinic pharmacist to care and make necessary nursing judgments. Responsible for systematically assessing the healthcare needs of the patients receiving warfarin and/or other anticoagulants. Demonstrates the ability to make decisions by integrating didactic knowledge with practical experience. Performs continual assessment, outcomes identification, planning, implementation, and evaluation of the care plan for anti-coagulated patients. Routine clinical management activities will include: INR result interpretation; warfarin dosage adjustment; follow up INR and/or other testing recommendations; ongoing assessment of anticoagulation efficacy; identification/management of potential complications; parenteral bridging assessment; peri-procedural anti-coagulation care planning; and continued individualized patient orientation to the anti-coagulation clinic.
Records notes, assessment data into EPIC system, creates appropriate visit notes and AVS (after visit summary).
Continually assesses patient and family educational needs with regard to anti-coagulation. Demonstrates an awareness of environmental, cultural, social, and/or financial factors on the anti-coagulation care of patients and their families. Provides anti-coagulation care in a manner that incorporates patient satisfaction principles and cost effective care. Develops relationships with patients and families that maintain and communicate trust and respect. Communicates effectively in the exchange of information. Demonstrates the ability to act as a patient advocate. Utilization of SEH multi-disciplinary resources for medical, financial, social and/or other assistance; identification of patient specific resources to improve anti-coagulation adherence; and information provision in the format that best allows patients to manage their specific anti-coagulation care needs.
Multi-disciplinary Collaboration and program development - Works in a collegial and collaborative relationship with other healthcare professionals to determine the healthcare needs of anti-coagulated patients.
Maintains annual mandatory education requirements, which include emergency skills and unit based competencies. Additional continuing education activities specific to anti-coagulation care will be completed on an annual basis following SEH policies. Identifies individual needs for continued growth and development in conjunction with the anti-coagulation pharmacist, staff and pharmacy supervisor. Obtains required bi-annual CEU’s including all state required courses.
Participates in the development, implementation and evaluation of the anti-coagulation clinic and/or SEH policies, procedures, quality improvement, research initiatives, and/or standards of practice.
Attends, and participates in anti-coagulation clinic staff meetings as appropriate.
Performs other duties as assigned
Qualifications
MINIMUM
DESIRABLE

Education, Credentials, Licenses:
Graduate of accredited school of nursing
Current license in the state where work is being performed
Anti-Coagulation Certification completion within 6 months of employment
Certified in Anti-coagulation therapy.

Specialized Knowledge:
Current CPR certification
Basic knowledge of nursing theory procedures and policies
Ability to perform finger stick collections

Kind and Length of Experience:
Two years of experience including anti-coagulation clinic or cardiology experience.
