The ALS Navigator is responsible for developing professional and community relationships that will enable the Association to provide care and support to those living with ALS. As the ALS Navigator, you will be responsible for the management of care services program, which may include, but is not limited, to ensuring the uniform delivery of high-quality, consistent services for individuals with ALS and their families. The ALS Navigator will also facilitate education, information, benefit, and clinical resource navigation assistance to persons diagnosed with ALS and caregivers.

DUTIES AND RESPONSIBILITIES:

  • Receive referrals and obtains all necessary patient, medical, demographic information, and documents from referral sources via various communication methods to establish and coordinate a plan of care.
  • Conduct specialized care consultations and needs assessments for people living with ALS caregivers and families in a variety of settings, including virtual visits, by phone/email, in ALS clinic, at support groups, and at home visits, when appropriate.
  • Actively manage a portfolio of patients, assuring patient and caregiver needs are met within the capabilities of The Association
  • Provide information on services and resources and printed/electronic educational materials available to persons with ALS and their families.
  • Coordinate referrals to ALS Association programs as well as healthcare providers and community agencies.
  • Facilitate resource support group meetings, engage participants, and schedule speakers as appropriate to meet the needs of the group.
  • Participate in continuing education seminars, workshops and conferences as available and necessary.
  • Work with Managing Director Care Services/Care Services Manager, and care services staff, to manage relationships with each multidisciplinary clinic and maintain strong communication with clinic staff.
  • Collaborate with the Managing Director Care Services to evaluate care services programs for effectiveness and desired outcomes, offering professional opinion/recommendations concerning current or future Care Services Programs
  • Ensure database is used consistently within territory and established standard operating procedures are followed within territory.
  • Ensure reports, including statistical, outcome, and other metrics are accurate and timely.
  • Offer recommendations concerning current or future programs.
  • Work with Managing Director Care Services/Care Services Director to establish strategic community and professional partnerships and collaborations.
  • Work with Managing Director Care Services/ Care Services Director to identify and resolve operational and service barriers within the territory.
  • Assist Managing Director Care Services/Care Service Director as assigned.
  • Participate in fundraising activities to include but not limited to providing client/caregiver prospects for Walk and event setup and day of activities.
  • Ability to travel as required or requested.
  • Others duties as assigned.

SUPERVISORY RESPONSIBILITIES:

  • This position does not currently have supervisory responsibilities. However, it is possible that supervisory responsibilities may be added based on the needs of the position and the Association.

When supervisory responsibilities are assigned, the requirements are as follow:

  • As required/requested supervises remote and office-based care services staff in the delivery of programs and services to individuals impacted by ALS and their families
  • As required/requested work with the Managing Director Care Services/Care Services Director to recruit, coach, mentor, train, and perform annual performance reviews of care services personnel
  • As applicable conducts regular 1:1 meeting with assigned care services staff

QUALIFICATIONS:

  • Bachelor’s Degree in social work, health administration, public health, nursing, or a related area required, Master’s preferred.
  • Minimum of three to five years of related professional experience, including direct care and case management
  • Ability to assess both physical and psychosocial needs of ALS client and families, based on knowledge of ALS, the disease, and its ramifications.
  • Experience with progressive neurodegenerative disease
  • Knowledge of Medicare, Medicaid, community resources, patient assessments skills, planning and case management preferred
  • Skilled communicator, with superior interpersonal, verbal, and written skills
  • Able to handle confidential information and maintain high confidentiality
  • Excellent organization skills and attention to detail
  • Display strong work ethic, personal integrity, empathy.
  • Proficiency in Microsoft Office Suite. Salesforce and database experience a plus
  • Ability to work independently with little supervision; must be a self-starter
  • Ability to manage multiple projects simultaneously
  • Excellent customer service skills
  • Ability to drive locally and travel nationally on Association business is required.
  • Ability to work occasional evenings and/or weekends for Assoc. business and events.

The ALS Association's pay range for this position is $65,397 - $70,932 annually.

The ALS Association has compensation ranges for various work locations throughout the United States, allowing us to compensate employees competitively and consistently in diverse geographic markets. The above-listed listed range shows prospective compensation for this role; the appropriate range will be established by what is eventually determined to be the candidate's primary work location. Various factors, including the complexity and significance of the role, job duties/requirements, relevant experience and abilities, and primary work location, determine individual compensation. Compensation ranges are evaluated and typically adjusted on a regular basis. Offers are made within the compensation range that is in effect at the time of the job offer.

The Association may, at its discretion, offer merit-based increases tied to individual performance. The benefits listed may differ depending on employment status with the Association.
